【摘要】：目的觀察嬰兒髖關(guān)節(jié)發(fā)育不良(DDH)篩查結(jié)果以及評(píng)價(jià)超聲檢查技術(shù)在早期診斷中的價(jià)值。方法選取2014年9月-2016年6月該院1 500例健康體檢嬰兒(3 000個(gè)髖關(guān)節(jié)),觀察髖關(guān)節(jié)發(fā)育不良高危因素(單雙胎、胎位不正、長(zhǎng)期捆綁及難產(chǎn)等)、大腿皮紋或臀紋、外展試驗(yàn)等臨床篩查內(nèi)容、超聲檢查結(jié)果以及Graf法分型等。結(jié)果臨床篩查發(fā)育不良378例,發(fā)育不良率25.20%;超聲檢查結(jié)果發(fā)育不良者22例(髖關(guān)節(jié)44個(gè)),發(fā)育不良率5.82%。女嬰發(fā)育不良率高于男嬰,左髖異常率高于右髖發(fā)育不良率,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。臨床篩查發(fā)育不良中捆綁雙下肢因素所占比例最高,與其他因素比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05),其次為DDH陽(yáng)性家族史,差異有統(tǒng)計(jì)學(xué)意義(P0.05),超聲診斷發(fā)育不良率,與臨床篩查一致。α角與年齡呈正相關(guān)(r=0.104,P0.050),β角與年齡呈負(fù)相關(guān)(r=-0.116,P0.050)。結(jié)論加強(qiáng)嬰兒DDH篩查,有助于掌握本地區(qū)該病流行病學(xué)資料、早期開展干預(yù),降低致殘率,且在篩查中,超聲檢查能提高嬰兒發(fā)育性髖關(guān)節(jié)發(fā)育不良診斷率,可作為臨床常規(guī)的篩查手段,值得推廣。
[Abstract]:Objective to observe the results of DDH screening in infants with hip dysplasia and to evaluate the value of ultrasonography in early diagnosis.Methods from September 2014 to June 2016, 1 500 healthy infants with 3 000 hip joint defects were selected to observe the high risk factors of hip dysplasia (single twin, abnormal fetal position, long term binding and dystocia), thigh dermatoglyphics or gluteal stripes.Outreaching test and other clinical screening, ultrasound results and Graf classification.Results there were 378 cases of dysplasia and 25.20% of dysplasia by clinical screening, and 22 cases of dysplasia (44 cases of hip joint, 5.82% of dysplasia) by ultrasonography.The maldevelopment rate of female infants was higher than that of male infants, and the abnormal rate of left hip was higher than that of right hip. The difference was statistically significant (P 0.05).In the clinical screening of dysplasia, the proportion of the factors of binding both lower limbs was the highest. Compared with other factors, the difference was statistically significant (P 0.05), followed by the DDH positive family history (P 0.05), and the rate of ultrasonic diagnosis of dysplasia.There was a positive correlation between 偽 angle and age (P 0.050) and a negative correlation between 尾 angle and age (P 0.050).Conclusion strengthening the screening of infant DDH is helpful to grasp the epidemiological data of the disease, to carry out early intervention and to reduce the rate of disability. In the screening, ultrasonic examination can improve the diagnosis rate of developmental hip dysplasia in infants.Can be used as a clinical routine screening means, worthy of promotion.
