【摘要】：目的： 采取隨機(jī)分組對(duì)照實(shí)驗(yàn)的方法,通過兩組不同的干預(yù)方案對(duì)早產(chǎn)兒行早期干預(yù),觀察早產(chǎn)兒的心身發(fā)育水平,對(duì)益腎健脾按摩組與嬰兒撫觸組行療效比較,評(píng)價(jià)中醫(yī)之“益腎健脾按摩”的可行性。 方法： 本課題采用分組對(duì)照實(shí)驗(yàn)方法,按照納入標(biāo)準(zhǔn)收入病例82例,對(duì)納入病例進(jìn)行隨機(jī)分組,試驗(yàn)組和對(duì)照組各41例。兩組均予早期教育的基礎(chǔ)上,試驗(yàn)組予益腎健脾按摩法,對(duì)照組予嬰兒撫觸法。分別在干預(yù)前,干預(yù)1個(gè)月結(jié)束、干預(yù)3個(gè)月結(jié)束時(shí)進(jìn)行評(píng)測。主要評(píng)估體格發(fā)育指標(biāo),體重、身長、頭圍；神經(jīng)心理發(fā)育指標(biāo),大運(yùn)動(dòng)、精細(xì)運(yùn)動(dòng)、適應(yīng)能力、語言、社交行為相當(dāng)月齡值。將所得的全部資料在Excel上建立數(shù)據(jù)文件,數(shù)據(jù)應(yīng)用SPSS13.0軟件進(jìn)行統(tǒng)計(jì)分析。 成果： (1)干預(yù)治療前兩組間的基本情況,入組時(shí)糾正月齡、體重、身長、頭圍、20項(xiàng)新生兒行為神經(jīng)測定(neonatal behavioral neurological assessment, NBNA)測量值,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。 (2)干預(yù)3個(gè)月后,兩組前后對(duì)比,體格發(fā)育水平及神經(jīng)心理發(fā)育商均高于干預(yù)1月后,且兩組分別行前后對(duì)比有顯著性差異(P0.01) (3)干預(yù)3個(gè)月后,兩組間對(duì)比,試驗(yàn)組體格發(fā)育指標(biāo)(身高、體重、頭圍)的總體均數(shù)高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)；試驗(yàn)組神經(jīng)心理發(fā)育指標(biāo),大運(yùn)動(dòng)、精細(xì)運(yùn)動(dòng)、適應(yīng)能力、語言、社交行為的相當(dāng)月齡值總體均數(shù)高于對(duì)照組,但這五項(xiàng)的組間差異無統(tǒng)計(jì)學(xué)意義(P0.05)。 結(jié)論： 益腎健脾按摩與嬰兒撫觸早期干預(yù)法皆能促進(jìn)早產(chǎn)兒體格與神經(jīng)心理發(fā)育,在干預(yù)3月后,益腎健脾按摩法在促體格發(fā)育方面(體重,身長,頭圍)療效比嬰兒撫觸干預(yù)法更顯著；在神經(jīng)心理發(fā)育,組間對(duì)比無明顯差異。
[Abstract]:Objective:. A randomized controlled trial was conducted to observe the psychosomatic development of premature infants by two different intervention schemes. The therapeutic effects were compared between the massage group of tonifying kidney and strengthening spleen and the touching group of infants. To evaluate the feasibility of "tonifying kidney and strengthening spleen massage" of traditional Chinese medicine. Methods:. In this study, 82 cases of standard income were randomly divided into two groups, 41 cases in the experimental group and 41 cases in the control group. On the basis of early education, the experimental group was given the method of tonifying the kidney and strengthening the spleen, and the experimental group was given the massage method of tonifying the kidney and strengthening the spleen. The control group was treated with infant touch method. Before the intervention, the intervention ended at the end of 1 month, and the intervention was evaluated at the end of 3 months. The indexes of physical development, body weight, body length, head circumference, neuropsychological development index, large movement, fine movement were evaluated. Adaptive ability, language, and social behavior are equivalent months of age. The data files are built on Excel, and the data are analyzed by SPSS13.0 software. Outcome:. 1) the basic condition of the two groups before the intervention treatment was corrected when entering the group. There was no significant difference in 20 neonatal behavioral nerve measurements (NBNAs), such as age, weight, body length, head circumference, and neonatal behavioral nerve assessment (NBNAs). There was no significant difference between the two groups (P 0.05). (2) after 3 months of intervention, the physical development level and neuropsychological development quotient of the two groups were higher than those after one month of intervention, and there was a significant difference between the two groups before and after the intervention (P 0.01). 3) after 3 months of intervention, the total mean of physical development indexes (height, weight, head circumference) in the experimental group was higher than that in the control group, and the difference was statistically significant (P 0.05). The total mean of age of adaptation, language and social behavior was higher than that of control group, but there was no significant difference among the five groups (P 0.05). Conclusion:. Both Yishen Jianpi massage and early infant touch intervention can promote physical and neuropsychological development of preterm infants. After 3 months of intervention, Yishen Jianpi massage can promote physical development (body weight, length, body length). Head circumference) was more effective than infant touch intervention, but there was no significant difference in neuropsychological development between groups.
