血管迷走性暈厥兒童血清鐵、血清鐵蛋白的變化及臨床意義

【摘要】:目的研究血管迷走性暈厥(VVS)兒童血清鐵(SI)、血清鐵蛋白(SF)的變化及臨床意義,探討其在兒童VVS發(fā)病機(jī)制中的作用。 方法2011年5月~2012年4月在中南大學(xué)湘雅二醫(yī)院兒童暈厥專科門診就診的不明原因暈厥(UPS)兒童,并經(jīng)直立傾斜試驗(yàn)(HUTT)確診為VVS的兒童21例為暈厥組,男12例,女9例,年齡8-14歲,平均(11.71±2.31)歲。選取性別、年齡匹配的18例健康兒童為對(duì)照組,男9例,女9例,年齡7~14歲,平均年齡(10.78±2.18)歲。對(duì)暈厥組兒童及對(duì)照組兒童在禁食禁飲12h后抽取肘靜脈血分別測(cè)定血清鐵(SI)、血清鐵蛋白(SF)、血紅蛋白(Hb)、血細(xì)胞比容(Hct)、平均紅細(xì)胞體積(MCV)。將所得數(shù)據(jù)根據(jù)不同分類進(jìn)行比較,用統(tǒng)計(jì)軟件SPSS16.0進(jìn)行數(shù)據(jù)統(tǒng)計(jì)分析。 結(jié)果①一般資料比較:兩組年齡(t=-1.32,P0.05)和性別(x2=0.199,P0.05)比較差異無(wú)統(tǒng)計(jì)學(xué)意義。②暈厥組與對(duì)照組比較,SI降低(15.15±4.55vs18.24±4.15μmol/L,P0.05).SF降低(59.10±26.81vs79.83±33.58μg/L,P0.05).Hct升高(40.19±3.44vs38.01±2.68%,P0.05)、MCV降低(84.85±5.80vs86.18±3.42fl,P0.05)、Hb降低(127.57±10.62vs132.29±12.57g/LP0.05)。③暈厥組不同性別SI、SF、Hct、MCV、Hb比較,男性稍高于女性(分別是15.34±3.26vs15.02±5.47μmol/L,P0.05:62.37±25.50vs54.76±29.42μg/L,P0.05;40.98±3.91vs39.14±2.50%,P0.05;85.47±4.60vs84.01±7.32fl,P0.05;134.67±13.49vs129.11±11.17g/L,P0.05).④反復(fù)暈厥組較偶發(fā)暈厥組SI、SF稍增高(15.19±4.37vs14.58±4.87μmol/L,P0.05:61.12±29.62vs57.27±25.30μg/L,P9.05),Hb.Hct.MCV稍降低(130.90±14.00vs133.55±11.65g/L,P0.05;40.12±3.73vs40.20±3.32%,p0.05;83.81±2.72vs85.99±7.98f1,P0.05). 結(jié)論①VVS兒童SI、SF顯著低于對(duì)照組;②VVS兒童SI、SF性別之間差異無(wú)統(tǒng)計(jì)學(xué)意義。③SI、SF對(duì)估測(cè)VVS兒童暈厥頻次無(wú)預(yù)測(cè)價(jià)值。
[Abstract]:Objective to study the changes and clinical significance of serum ferritin in children with vasovagal syncope (VVS) and to explore its role in the pathogenesis of VVS in children. Methods from May 2011 to April 2012, 21 children (12 males and 9 females) with VVS diagnosed by orthostatic tilt test in Xiangya second Hospital of Central South University were selected as syncope group. The age ranged from 8 to 14 years (mean 11.71 鹵2.31) years. 18 healthy children with sex and matched age were selected as control group, including 9 males and 9 females, aged 7 to 14 years. The mean age was 10.78 鹵2.18 years. The blood samples from elbow vein were collected from children in syncope group and control group after fasting for 12 hours. The blood samples were collected for the determination of serum iron siella, serum ferritin, hemoglobin hbtl, hematocrit, mean erythrocyte volume and MCV. Comparing them according to different classifications, Statistical software SPSS16.0 was used to analyze the data. Results 1 comparison of general data: there was no significant difference between the two groups in age and sex (P 0.05) and sex (20.199P0.05). 2 the SI of syncope group decreased 15.15 鹵4.55 vs 18.24 鹵4.15 渭 mol / L P 0.05. Conclusion (1) there was no significant difference in Sisf between males and females in VVS children compared with the control group (2VVS). 3SI-SF had no predictive value in estimating the frequency of syncope in children with VVS.
