【摘要】：目的探討缺血修飾蛋白(IMA)對肺炎合并心力衰竭新生兒心肌損傷程度及預后的意義。方法選取肺炎、肺炎合并心力衰竭及正常新生兒各60例,檢測其血清IMA、肌鈣蛋白I(c Tn I)和肌酸激酶同工酶(CKMB)水平并進行比較。通過超聲檢測計算3組新生兒左心室心肌做功指數(Tei指數)并進行比較。分析肺炎合并心力衰竭組新生兒IMA與c Tn I、CK-MB及左心室Tei指數的相關性。隨訪肺炎合并心力衰竭組新生兒,記錄該組患兒轉歸情況,分析IMA水平與預后的關系。結果 3組患兒的IMA、c Tn I和CK-MB差異有統(tǒng)計學意義(P0.05),肺炎合并心力衰竭組患兒IMA為(96.84±10.49)U/m L,c Tn I為(1.40±0.30)μg/L,CK-MB為(69.66±13.2)U/L,上述3種指標均顯著高于其余兩組(P0.05)。肺炎合并心力衰竭組左心室Tei指數為0.43±0.12,顯著高于其余兩組(P0.05)。肺炎合并心力衰竭組新生兒IMA與c Tn I、CK-MB及左心室Tei指數均呈顯著正相關(r=0.81、0.73、0.69,P0.05)。肺炎合并心力衰竭組高IMA水平轉歸情況包括心律失常及死亡。結論缺血修飾蛋白可作為肺炎合并心力衰竭新生兒心肌損害程度及預后判斷的有效指標。
[Abstract]:Objective to investigate the significance of ischemia modified protein (IMA) in myocardial injury and prognosis of neonates with pneumonia and heart failure. Methods 60 cases of pneumonia complicated with heart failure and 60 cases of normal neonates were selected. The levels of serum IMA, cardiac troponin I (TnI) and creatine kinase isoenzyme (CKMB) were measured and compared. The correlation between IMA and CK-MB and Tei index of left ventricle in neonates of failure group. The newborns in pneumonia complicated with heart failure group were followed up. The outcome of the group was recorded, Results the difference of IMAc TnI and CK-MB was statistically significant among the three groups, and the IMA of pneumonia with heart failure was 96.84 鹵10.49 渭 g / m TnI (1.40 鹵0.30) 渭 g / L + CK-MB = 69.66 鹵13.2 渭 g / L, respectively. These three indexes were significantly higher than those of the other two groups. The Tei index of left ventricle in pneumonia with heart failure group was 0.43 鹵0.12, which was significantly higher than that in the other two groups. There was a significant positive correlation between IMA and c TNI CK-MB and left ventricular Tei index in pneumonia with heart failure group. Conclusion Ischemic modified protein can be used as an effective indicator of myocardial damage and prognosis in neonates with pneumonia and heart failure.
