【摘要】：目的:比較腎病綜合征患兒血清同型半胱氨酸水平變化并觀察其血同型半胱氨酸濃度與血漿白蛋白的相關(guān)性,分析其原因及臨床意義。方法:收集2016年1月至2017年2月就診于山西醫(yī)科大學(xué)和山西省兒童醫(yī)院首次確診為腎病綜合征的患兒30例,同時(shí)選取非腎病綜合征兒童30例,于清晨空腹抽取4ml血液,然后采用ELISA檢測兩組兒童血清同型半胱氨酸水平,并檢測患兒血清白蛋白濃度;采用SPSS 18.0分析兩組兒童血清Hcy的變化以及患兒Hcy與白蛋白之間的關(guān)系;最后結(jié)合臨床,分析數(shù)據(jù)意義。結(jié)果:1.腎病綜合征患兒中有血清Hcy濃度的均數(shù)為11.310±3.568μmol/L,對照組中為7.373±2.444μmol/L,兩組數(shù)據(jù)經(jīng)協(xié)方差分析后得P0.001,差別具有統(tǒng)計(jì)學(xué)意義;2.腎病綜合征患兒血漿Hcy濃度與白蛋白呈正相關(guān)。結(jié)論:1.HHcy不僅可以導(dǎo)致腎小球硬化,而且還可以促進(jìn)血栓形成,而本研究發(fā)現(xiàn)腎病綜合征患兒體內(nèi)存在不同程度的HHcy,因此減少Hcy濃度可能延緩腎臟損害,降低NS患兒腎靜脈血栓形成的幾率。2.Hcy在血漿中大部分是以與白蛋白結(jié)合的結(jié)合型存在,血清Hcy與白蛋白濃度呈正相關(guān),所以由于尿蛋白排出導(dǎo)致的低蛋白血癥可能影響血漿Hcy濃度。
[Abstract]:Objective: to compare the changes of serum homocysteine level in children with nephrotic syndrome and to observe the correlation between serum homocysteine concentration and plasma albumin. Methods: from January 2016 to February 2017, 30 children with nephrotic syndrome were selected from Shanxi Medical University and Shanxi Children's Hospital, and 30 children with non-nephrotic syndrome were selected. The serum homocysteine level and serum albumin concentration were measured by ELISA, the changes of serum Hcy and the relationship between Hcy and albumin in the two groups were analyzed by SPSS 18.0. Finally, combined with clinical practice, The mean of serum Hcy concentration in children with nephrotic syndrome was 11.310 鹵3.568 渭 mol / L, and that in control group was 7.373 鹵2.444 渭 mol / L. The results of covariance analysis showed that the difference was statistically significant (P 0.001). The plasma Hcy concentration in children with nephrotic syndrome was significantly higher than that in control group (P < 0.05). Conclusion: 1. HHcy can not only cause glomerulosclerosis, This study also found that children with nephrotic syndrome had different levels of Hcy, so reducing the concentration of Hcy might delay renal damage. Reducing the probability of renal venous thrombosis in NS children. 2. Most of Hcy in plasma is bound to albumin, and serum Hcy is positively correlated with albumin concentration. So hypoproteinemia due to urinary protein excretion may affect plasma Hcy concentration.
