【摘要】：目的 探討鈦制彈性髓內(nèi)釘治療兒童脛腓骨骨折的手術(shù)技巧、臨床療效、適應(yīng)癥、并發(fā)癥及注意事項(xiàng)，，為兒童脛腓骨骨折提供一種切實(shí)可行、行之有效的治療方法。 方法 回顧性分析我院2008年1月-2013年11月在C形臂X線機(jī)監(jiān)視下采用AO鈦制彈性髓內(nèi)釘（TEN）治療兒童脛腓骨骨折患兒59例（男43例，女16例），年齡2～14歲，平均6歲。術(shù)后所有病例均進(jìn)行隨訪，隨訪內(nèi)容包括骨折愈合情況、傷口情況、肢體長度、膝踝關(guān)節(jié)活動范圍以及步態(tài)等情況。 結(jié)果 所有患兒隨訪6～36個(gè)月，平均16個(gè)月。所有骨折均愈合，無嚴(yán)重并發(fā)癥發(fā)生。術(shù)后無1例出現(xiàn)感染、釘尾處紅腫疼痛、TEN折斷、骨折不愈合、骨骺損傷、下肢各關(guān)節(jié)功能異常等并發(fā)癥。根據(jù)Flynn評價(jià)標(biāo)準(zhǔn)，優(yōu)49例，良10例，優(yōu)良率達(dá)到100％。 結(jié)論 TEN固定治療兒童脛腓骨骨折具有軟組織損傷小、骨折端血運(yùn)破壞少、不良反應(yīng)少、材質(zhì)優(yōu)良、穩(wěn)定性好、骨折愈合快、住院時(shí)間短、并發(fā)癥少等多種優(yōu)點(diǎn)，是治療兒童脛腓骨骨折的理想方法之一。
[Abstract]:Purpose. To explore the operative technique, clinical effect, indications, complications and precautions of titanium elastic intramedullary nail in the treatment of tibia and fibula fracture in children, and to provide a feasible and effective treatment method for tibia and fibula fracture in children. Method. 59 children with tibia and fibula fractures were treated with AO titanium elastic intramedullary nail (TEN) under the supervision of C-arm X-ray machine from January 2008 to November 2013 in our hospital (43 males and 16 females, aged 214 years). The average age was 6 years. All cases were followed up including fracture healing, wound condition, limb length, range of knee and ankle joint movement and gait. Results. All the children were followed up for 6 ~ 36 months with an average of 16 months. All the fractures healed without serious complications. According to Flynn evaluation criteria, 49 cases were excellent, 10 cases were good, and the excellent and good rate was 100%. Conclusion. The treatment of tibia and fibula fracture with TEN has many advantages, such as little soft tissue injury, less blood damage, less adverse reaction, good material quality, good stability, quick fracture healing, short hospital stay, less complications and so on. It is an ideal method for the treatment of tibia and fibula fracture in children.
