【摘要】：目的:泄瀉為兒科常見(jiàn)病多發(fā)病,為兒科重點(diǎn)防治四病之一。香訶止瀉方系導(dǎo)師熊磊用于治療小兒脾虛泄瀉的經(jīng)驗(yàn)方,本研究對(duì)該方臨床療效、安全性及作用機(jī)制進(jìn)行初步評(píng)價(jià),為研制醫(yī)院制劑提供前期工作基礎(chǔ)。方法:1.臨床研究:在云南省中醫(yī)醫(yī)院門(mén)診收集符合脾虛泄瀉納入標(biāo)準(zhǔn)的60例患兒,采用香訶止瀉方口服治療6天,自身前后對(duì)照觀察給藥后患兒癥狀積分變化以及有效率。2.實(shí)驗(yàn)研究:通過(guò)單次給藥毒性實(shí)驗(yàn)評(píng)價(jià)香訶止瀉方安全性;通過(guò)小鼠胃排空及腸推進(jìn)實(shí)驗(yàn),對(duì)該方作用機(jī)制進(jìn)行初步探討。結(jié)果:1.臨床研究:治療后積分較治療前明顯降低,臨床有效率達(dá)90%,對(duì)主要癥狀有效率達(dá)90%,次要癥狀有效率達(dá)83.3%。2.實(shí)驗(yàn)研究:香訶止瀉方小鼠的日最大給藥量為440.01g/kg(生藥),約成年人(60kg)日臨床用量(1.83g/kg)的240倍。小鼠胃排空實(shí)驗(yàn)及腸推進(jìn)實(shí)驗(yàn)中,硫酸阿托品及香訶止瀉方高劑量組能提高胃殘留率和抑制腸推進(jìn),與正常對(duì)照組相比差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:香訶止瀉方對(duì)脾虛型泄瀉治療療效確切,且對(duì)脾虛泄瀉帶來(lái)諸癥腹瀉次數(shù)、大便性狀、病程、嘔吐、腹痛、口渴、納差等均有明顯改善作用。通過(guò)實(shí)驗(yàn)結(jié)果推測(cè)藥物止瀉作用機(jī)制可能與抑制胃腸蠕動(dòng)有關(guān)。本研究為臨床應(yīng)用香訶止瀉方治療脾虛型泄瀉提供了依據(jù)。
[Abstract]:Objective: diarrhea is one of the most common diseases in pediatrics and one of the most important four diseases in pediatrics. Xiong Lei, a tutor of XianghaZhixie recipe, was used to treat diarrhea due to spleen deficiency in children. Methods: 1. Clinical study: 60 children who met the criteria of diarrhea due to spleen deficiency were collected from outpatient clinic of Yunnan Provincial traditional Chinese Medicine Hospital. After 6 days of oral treatment with Xianghazhixie recipe, the changes of symptom score and effective rate were observed before and after administration. 2. Experimental study: the safety of Xianghe Zhixie recipe was evaluated by single dose toxicity experiment; The action mechanism of this prescription was preliminarily discussed by gastric emptying and intestinal propulsion test in mice. Results: 1. Clinical study: the score after treatment was significantly lower than that before treatment. The clinical effective rate was 90, the effective rate for main symptoms was 90, and the effective rate for secondary symptoms was 83.3. 2. Experimental study: the maximum daily dose of Xianghazhixie recipe in mice was 440.01g / kg. The gastric emptying of mice was 240 times as much as the daily clinical dosage of 1.83 g / kg. In experiments and bowel propulsion experiments, The high dose group of atropine sulfate and Xianghazhixie prescription could increase the gastric residual rate and inhibit the intestinal propulsion, and the difference was statistically significant compared with the normal control group (P 0.05). Conclusion: Xianghazhixie recipe is effective in treating diarrhea of spleen deficiency type. And the number of diarrhea, defecation traits, course of disease, vomiting, abdominal pain, thirst caused by diarrhea due to spleen deficiency, According to the experimental results, the mechanism of antidiarrhea may be related to the inhibition of gastrointestinal peristalsis. This study provides a basis for the clinical application of XianghaZhixie recipe in the treatment of diarrhea due to spleen deficiency.
