【摘要】：目的觀察β受體阻滯劑對(duì)小兒擴(kuò)張型心肌病(DCM)的療效。方法對(duì)1995年至2015年在該院住院的DCM患兒61例進(jìn)行回顧性分析,根據(jù)是否服用β受體阻滯劑,將患兒分為治療組(服用β受體阻滯劑,n=35)和對(duì)照組(未服用β受體阻滯劑,n=26),初步觀察β受體阻滯劑的療效。結(jié)果治療后,治療組心率、左心室舒張末期內(nèi)徑(LVDD)、左心室收縮末期內(nèi)徑(LVSD)、左心房?jī)?nèi)徑(LA)較治療前明顯降低,射血分?jǐn)?shù)(EF)明顯升高(P0.05);對(duì)照組治療后LVDD明顯降低(P0.05),心率、右心室內(nèi)徑(RV)、LVDD、LVSD、LA、二尖瓣口血流速度(MV)及EF與入院前比較無明顯變化(P0.05)。治療后,治療組心率、LVDD、LVSD、EF與對(duì)照組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),治療組心功能較對(duì)照組明顯改善(P0.05)。治療組中伴有高血壓的患兒心功能改善程度明顯好于血壓正常患兒(P0.05)。治療組中經(jīng)卡維地洛治療的患兒心功能改善程度明顯好于美托洛爾治療的患兒(P0.05)。結(jié)論β受體阻滯劑可用于小兒DCM的治療,對(duì)伴有高血壓的DCM患兒療效更佳,其中卡維地洛比美托洛爾療效更加優(yōu)秀。
[Abstract]:Objective To observe the effect of beta blockers in children with dilated cardiomyopathy (DCM) patients. Methods from 1995 to 2015 in the hospital from 61 DCM patients were retrospectively analyzed, according to whether taking beta blockers, patients were divided into treatment group (beta blocker, n=35) and control group (not taking beta receptor blocker, n=26), preliminary observation on the effects of beta blockers. Results after treatment, the treatment group of heart rate, left ventricular end diastolic diameter (LVDD), left ventricular end systolic diameter (LVSD), left atrial diameter (LA) significantly reduced ejection fraction (EF) increased significantly (P0.05); the control group after treatment, LVDD decreased significantly (P0.05), heart rate, right ventricular diameter (RV), LVDD, LVSD, LA, mitral blood flow velocity (MV) and EF before admission, no significant difference (P0.05). After treatment, the treatment group of heart rate, LVDD, LVSD, there was significant difference between EF and the control group (P0.05) The treatment group, the heart function improved significantly compared with the control group (P0.05). Cardiac function in patients with hypertension associated with the treatment group improved significantly better than the normal blood pressure in children (P0.05). Heart function in children with treatment with carvedilol treatment group improved significantly better than metoprolol in the treatment of children (P0.05). Conclusion the beta receptor blocker for the treatment of DCM in children, the DCM children with hypertension curative effect is better than the effect of metoprolol, carvedilol is more excellent.
