【摘要】：目的:厭食是兒科常見病,長(zhǎng)期厭食可導(dǎo)致營(yíng)養(yǎng)不良、貧血等并發(fā)癥,長(zhǎng)期得不到治療會(huì)影響患兒的生長(zhǎng)發(fā)育。針對(duì)厭食,歷代中醫(yī)醫(yī)家多從脾胃角度論治。歷代醫(yī)家經(jīng)過大量的臨床也驗(yàn)證了調(diào)理脾胃是治療厭食的基本大法,尤其以補(bǔ)益脾氣為最常見治療方法。臨床上從脾腎氣虛證論治厭食的醫(yī)家僅有少數(shù)。導(dǎo)師通過研究發(fā)現(xiàn):臨床上從補(bǔ)腎健脾法治療脾腎氣虛型厭食可取得較滿意的療效,此類型厭食除脾腎氣虛外,常伴有食積、濕濁、郁熱等邪氣內(nèi)停。治療上除補(bǔ)益脾腎之氣外,還輔以行氣化濕、消食清熱。本課題擬通過對(duì)健脾補(bǔ)腎方治療脾腎氣虛型厭食進(jìn)行系統(tǒng)性、規(guī)范性的研究,從而為開闊治療厭食的方法思路、提高厭食治愈率提供客觀的依據(jù)。方法:采用隨機(jī)對(duì)照實(shí)驗(yàn)的方法,將受試按預(yù)先確定的隨機(jī)數(shù)分配到以下兩組:治療組:內(nèi)服自擬"補(bǔ)腎健脾方"方:菟絲子、補(bǔ)骨脂、白術(shù)、黨參、炙甘草、厚樸花、法半夏、連翹、神曲。對(duì)照組:厭食顆粒。治療時(shí)間為2周。治療前、治療后各記錄一次積分。治療2周結(jié)束時(shí)進(jìn)行療效評(píng)價(jià)。結(jié)果:①治療后兩組患者總有效率比較,差異有統(tǒng)計(jì)學(xué)意義(P=0.040.05),提示治療組療效優(yōu)于對(duì)照組。②治療后,兩組患兒中醫(yī)證候總分比較,差異有統(tǒng)計(jì)學(xué)意義(P=O.010.05),提示治療組中醫(yī)證候改善優(yōu)于對(duì)照組。③兩組各項(xiàng)證候積分對(duì)比:在食量、四肢膚溫、大便評(píng)分比較,治療組患兒改善優(yōu)于對(duì)照組。在面色、精神評(píng)分方面比較,兩組患兒證候改善療效相當(dāng)。結(jié)論:補(bǔ)腎健脾方具有補(bǔ)益脾腎、行氣化濕、消食清熱之功效。治療脾腎氣虛證厭食具有較好的療效,值得臨床推廣。
[Abstract]:Objective: anorexia is a common disease in pediatrics. Long-term anorexia can lead to malnutrition, anemia and other complications. Chinese medicine doctors of all dynasties have mostly treated them from the point of view of spleen and stomach. After a large number of clinical experiences, doctors have also proved that regulating the spleen and stomach is the basic method of treating anorexia. There are only a few doctors who treat anorexia from spleen and kidney qi deficiency syndrome in clinic. The tutor found that the method of tonifying kidney and invigorating spleen can obtain satisfactory curative effect in treating anorexia of spleen and kidney qi deficiency in clinic, especially in treating anorexia of deficiency of spleen and kidney qi by the method of tonifying spleen and strengthening spleen, and treating anorexia with deficiency of spleen and kidney qi. This type of anorexia, in addition to deficiency of spleen and kidney qi, often accompanied by accumulation of food, dampness, stagnation and heat, etc. In addition to tonifying the spleen and kidney, it is accompanied by vaporizing dampness. This subject aims to make a systematic and normative study on the treatment of deficiency of qi and deficiency of spleen and kidney by the prescription of invigorating the spleen and tonifying the kidney, so as to open the way for the treatment of anorexia. Methods: by means of randomized controlled trial, the subjects were assigned to the following two groups according to predetermined random numbers: the treatment group: take the recipe of tonifying kidney and invigorating spleen: Cuscuta chinensis, Psoralea psoraleae, et al. Atractylodes macrocephala, Codonopsis, Radix Glycyrrhizae, Magnolia officinalis, Rhizoma Pinelliae, Forsythia Forsythia, Shenqu. Control group: anorexia granules. Treatment time is 2 weeks. Results the total effective rate of the two groups after treatment was compared and the difference was statistically significant, indicating that the curative effect of the treatment group was better than that of the control group after 2 weeks of treatment. The difference of TCM syndromes between the two groups was statistically significant, indicating that the improvement of TCM syndromes in the treatment group was better than that in the control group. The improvement of the treatment group was better than that of the control group. In the aspect of facial color and mental score, the two groups had the same curative effect on the improvement of syndrome. Conclusion: Bushen Jianpi prescription has the function of tonifying the spleen and kidney, and taking the dampness of gasification. The effect of eliminating food and clearing away heat. Treating anorexia of spleen and kidney qi deficiency syndrome has better curative effect and is worth popularizing clinically.
