MTHFR、ABCB1基因多態(tài)性與急性淋巴細(xì)胞白血病患兒甲氨蝶呤化療毒副反應(yīng)的關(guān)系

【摘要】:目的:研究亞甲基四氫葉酸還原酶(MTHFR)1298、677位點及三磷酸腺苷結(jié)合盒轉(zhuǎn)運體B1(ABCB1)基因多態(tài)性與急性淋巴細(xì)胞白血病(ALL)患兒大劑量甲氨蝶呤(HDMTX)化療治療后發(fā)生毒副反應(yīng)的關(guān)系。方法:收集43例2015年2月至2016年12月新疆醫(yī)科大學(xué)第一附屬醫(yī)院的急性淋巴細(xì)胞白血病患兒外周血,用聚合酶鏈反應(yīng)(PCR)-芯片雜交技術(shù)檢測患兒MTHFR及ABCB1基因型,同時觀察接受HDMTX化療治療的ALL患兒發(fā)生的毒副反應(yīng)。結(jié)果:MTHFR 677CT+TT基因型患兒發(fā)生口腔黏膜損害、骨髓抑制的風(fēng)險較CC基因型者顯著性增高(P0.05);MTHFR 1298AA基因型患兒發(fā)生胃腸道反應(yīng)的風(fēng)險較AC+CC基因型者顯著性增高(P0.05);ABCB1 C3435T基因型為CT+TT的ALL患兒發(fā)生骨髓抑制毒副作用的風(fēng)險高于基因型為CC的患兒(P0.05)。結(jié)論:MTHFR、ABCB1基因多態(tài)性分布與ALL患兒HDMTX化療后發(fā)生毒副反應(yīng)有關(guān)。
[Abstract]:Objective: to study the relationship between the polymorphism of methylenetetrahydrofolate reductase MTHFRn1298F677 and adenosine triphosphate binding cassette transporter B1 (ABCB1) gene polymorphism in children with acute lymphoblastic leukemia (ALL) and the side effects of high dose methotrexate (HDMTX) chemotherapy in children with acute lymphoblastic leukemia (ALL). Methods: the peripheral blood of 43 children with acute lymphoblastic leukemia from February 2015 to December 2016 in the first affiliated Hospital of Xinjiang Medical University were collected. The genotypes of MTHFR and ABCB1 were detected by polymerase chain reaction (PCR) and PCR microarray hybridization, and the side effects of ALL treated with HDMTX chemotherapy were observed. Results the oral mucosal damage was observed in the children with the ALL genotype of 1: MTHFR677CT. The risk of bone marrow suppression is significantly higher than that of CC genotype. The risk of gastrointestinal reaction in children with MTHFR1298AA genotype is significantly higher than that in patients with AC CC genotype. The side effects of bone marrow suppression in patients with ALL with ABCB1 C3435T genotype of P0.05 or ABCB1 C3435T are significantly higher than those of patients with CT-TT genotype. Conclusion the distribution of ABCB1 gene polymorphism in children with ALL is associated with adverse effects of HDMTX chemotherapy.
