2014-2015年廣州759例兒童腸道病毒感染病原譜調(diào)查

【摘要】:目的調(diào)查2014-2015年我院759例兒童感染腸道病毒血清型的分布,了解廣州地區(qū)腸道病毒(enterovirus,EV)病原譜。方法采集手足口病等疑似腸道病毒感染患兒肛拭子標(biāo)本,采用熒光逆轉(zhuǎn)錄多聚酶鏈反應(yīng)法(reverse transcriptase polymerase chain reaction,RT-PCR)檢測(cè)通用型、腸道病毒71型(enterovirus 71,EV71)柯薩奇病毒A組16型(coxsackie virus A group 16,CA16)核酸,巢式PCR法擴(kuò)增VP1基因測(cè)序分型。結(jié)果 1 060例疑似感染者檢出759例EV陽性(檢出率71.6%);兩年均檢出11種血清型,2014年CA16、EV71、CA4、CA10和CA6位列前五位,CA16和EV71占77.1%;2015年CA6、EV71、CA10、CA16和CA2列前五位,CA6占51.7%;性別比為1.8~5.3;感染以3歲兒童為主(60.1%,456/759);全年均可檢出,4~7月達(dá)高峰,5月CA16、EV71、CA10和CA2達(dá)高峰,7月CA6達(dá)高峰。由于病原譜不同,2015年病情普遍較2014年輕,兩年均無死亡病例。EV71感染重癥病例比率最高(12.7%),CA16、CA10、CA4和CA6偶見重癥病例。結(jié)論廣州主要流行十余種腸道病毒,兩年病原譜不同,大部分感染引起疾病較輕。
[Abstract]:Objective to investigate the distribution of enterovirus serotypes in 759 children infected with enterovirus in our hospital in 2014-2015. Methods the anal swabs of children with suspected enterovirus infection such as hand, foot and mouth disease were collected. Reverse transcriptase polymerase chain reaction was detected by fluorescence reverse transcriptase polymerase chain reaction (FRT). RT-PCR was used to detect enterovirus 71. EV71) Coxsackie virus A group 16CA16) nucleic acid. VP1 genotyping was amplified by nested PCR. Results 759 EV-positive cases were detected in 1 060 suspected infected patients (the detection rate was 71.6%). In 2014, CA16 EV71, CA4, CA10 and CA6 ranked the top five, CA16 and EV71 accounted for 77.1%. In 2015, CA6 EV71, CA10, CA16, and CA2, the top five, accounted for 51.7 percent of the total. The sex ratio was 1.8 鹵5.3; The infection was mainly in children aged 3 years, 60.1% and 456 / 759%; During the whole year, the peak was detected from 4 to July, CA16 EV71, CA10 and CA2 reached its peak on May, and CA6 reached its peak on July, due to different pathogenetic profiles. In 2015, the disease was generally younger than that in 2014, and there were no death cases in two years. The rate of severe cases infected with EV71 was the highest (12.7%). CA4 and CA6 are rare severe cases. Conclusion there are more than ten kinds of enterovirus in Guangzhou, the pathogeny spectrum is different in two years, and most of the infections cause mild diseases.
