【摘要】：“醫(yī)療”問(wèn)題是一個(gè)民生問(wèn)題,它與社會(huì)公眾的切身利益直接相關(guān)。營(yíng)利性醫(yī)療機(jī)構(gòu)整個(gè)醫(yī)療行業(yè)的重要組成部分,是我國(guó)醫(yī)療市場(chǎng)的競(jìng)爭(zhēng)主體,是非營(yíng)利性醫(yī)療機(jī)構(gòu)的“同盟軍”。它的存在與發(fā)展不僅可以增強(qiáng)產(chǎn)業(yè)內(nèi)的競(jìng)爭(zhēng),提高整個(gè)產(chǎn)業(yè)效率,而且在滿(mǎn)足人民的醫(yī)療需求,減輕政府財(cái)政負(fù)擔(dān),促進(jìn)醫(yī)療改革等各方面都起著非常重要的作用。2009年新醫(yī)改以來(lái),國(guó)家相繼出臺(tái)了一系列重大利好政策,不斷放寬了營(yíng)利性醫(yī)療機(jī)構(gòu)的準(zhǔn)入,使其進(jìn)入市場(chǎng)變得容易了許多。然而現(xiàn)實(shí)并不樂(lè)觀,目前營(yíng)利性醫(yī)療機(jī)構(gòu)的在中國(guó)的發(fā)展仍受到極大的限制,這首先就體現(xiàn)在市場(chǎng)準(zhǔn)入制度方面。營(yíng)利性醫(yī)療機(jī)構(gòu)市場(chǎng)準(zhǔn)入制度作為政府管制市場(chǎng)醫(yī)療服務(wù)業(yè)的首要端口,與整理個(gè)醫(yī)療市場(chǎng)的健康發(fā)展息息相關(guān)。因此,對(duì)營(yíng)利性醫(yī)療機(jī)構(gòu)的市場(chǎng)準(zhǔn)入制度進(jìn)行研究,發(fā)現(xiàn)并完善其制定與執(zhí)行方面的不足,對(duì)進(jìn)一步促進(jìn)我國(guó)醫(yī)療市場(chǎng)的健康發(fā)展具有非常重要的意義。本文采用提出問(wèn)題——分析問(wèn)題——解決問(wèn)題的思路,首先對(duì)本研究課題的背景與意義、國(guó)內(nèi)外研究現(xiàn)狀、以及擬解決的問(wèn)題作簡(jiǎn)單概述;第二部分從一般性理論層面對(duì)營(yíng)利性醫(yī)療機(jī)構(gòu)準(zhǔn)入制度作簡(jiǎn)單介紹,對(duì)“營(yíng)利性醫(yī)療機(jī)構(gòu)”、“市場(chǎng)準(zhǔn)入”等相關(guān)概念進(jìn)行說(shuō)明,對(duì)發(fā)展簡(jiǎn)史進(jìn)行回顧,為后文的深入探討做鋪墊;第三部分從準(zhǔn)入條件、準(zhǔn)入程序、準(zhǔn)入方式這三方面的對(duì)我國(guó)營(yíng)利性醫(yī)療機(jī)構(gòu)準(zhǔn)入制度目前所存在的缺陷作了比較全面細(xì)致的闡述,并從政策目標(biāo)、管制機(jī)構(gòu)、配套監(jiān)管、以及準(zhǔn)入制度本身等四方面對(duì)其制度缺陷所產(chǎn)生原因進(jìn)行剖析;第四部分對(duì)美德日三國(guó)中一些值得借鑒的機(jī)制進(jìn)行介紹,并對(duì)其所提供的經(jīng)驗(yàn)與啟示進(jìn)行總結(jié);最后,針對(duì)當(dāng)前的問(wèn)題,在結(jié)合本國(guó)實(shí)際的基礎(chǔ)上從政策目標(biāo)、制度內(nèi)容、配套監(jiān)管等三方面提出了自己的改善建議。
[Abstract]:The issue of "medical care" is a livelihood issue, which is directly related to the vital interests of the public. Profit-making medical institutions are an important part of the entire medical industry and the main body of competition in China's medical market. Its existence and development can not only enhance competition within the industry and improve the efficiency of the entire industry, but also meet the medical needs of the people and lighten the financial burden of the government. Since the new health care reform in 2009, the country has introduced a series of major positive policies and has continuously relaxed access to for-profit medical institutions. It has made it much easier to enter the market. However, the reality is not optimistic. At present, the development of for-profit medical institutions in China is still severely restricted. This is reflected first in the market access system. As the primary port of the government to regulate the medical service industry in the market, the market access system for for-profit medical institutions is closely related to the healthy development of a medical market. The market access system of profit-making medical institutions is studied, and the deficiencies in its formulation and implementation are found and improved. It is of great significance to further promote the healthy development of the medical market in China. This paper adopts the idea of putting forward problem-analysis problem-solving problems, first of all, the background and significance of this research topic, the current research situation at home and abroad, The second part gives a brief introduction to the admission system of profit-making medical institutions from the general theoretical level, and explains the relevant concepts such as "for-profit medical institutions", "market access" and so on. The brief history of development is reviewed to pave the way for further discussion. The third part is from the access conditions, access procedures, These three aspects of access mode have made a more comprehensive and detailed exposition on the defects existing in the admission system of our country's for-profit medical institutions at present, and from the policy objectives, the regulatory agencies, the supporting supervision, As well as the access system itself to analyze the causes of its system defects; the fourth part of the three countries of the United States of Germany and Japan to draw lessons from the mechanism, and to provide a summary of their experience and inspiration; finally, In view of the current problems, this paper puts forward some suggestions for improvement from three aspects: policy goal, system content, supporting supervision and so on, on the basis of combining the actual situation of our country.
