人腦挫傷后HSP70、bFGF和COX2免疫組織化學(xué)研究

【摘要】: 前言 隨著醫(yī)學(xué)界對(duì)中樞神經(jīng)系統(tǒng)創(chuàng)傷后分子病理學(xué)認(rèn)識(shí)的提高以及免疫組織化學(xué)技術(shù)的進(jìn)步,人們從免疫組織化學(xué)角度,對(duì)腦挫傷與時(shí)間的關(guān)系做了大量的研究。由于受到檢材要求所限,在此方面的研究多為模擬腦挫傷的動(dòng)物實(shí)驗(yàn),從而造成了理論研究與實(shí)際鑒定工作相“脫鉤”的現(xiàn)象。 目前,已有人分別應(yīng)用免疫組織化學(xué)技術(shù)對(duì)熱休克蛋白70(HSP70)、堿性成纖維細(xì)胞生長(zhǎng)因子(bFGF)和環(huán)氧合酶-2(COX2)在大鼠腦挫傷后的表達(dá)進(jìn)行了研究,結(jié)果表明這三項(xiàng)指標(biāo)的表達(dá)在傷后均呈現(xiàn)出一定的變化規(guī)律,但未見(jiàn)有人將HSP70、bFGF和COX2應(yīng)用于人腦挫傷時(shí)間推斷的研究。故本實(shí)驗(yàn)采用實(shí)際工作中的案例標(biāo)本,應(yīng)用免疫組織化學(xué)染色,,旨在了解人腦挫傷后HSP70、bFGF和COX2的表達(dá),揭示其表達(dá)與人腦挫傷時(shí)間變化關(guān)系,同時(shí)觀察尸體解剖標(biāo)本與動(dòng)物實(shí)驗(yàn)標(biāo)本免疫組織化學(xué)的差別,為腦挫傷時(shí)間判定實(shí)際鑒定工作提供一條可行的理論依據(jù)。 實(shí)驗(yàn)材料與方法 1.實(shí)驗(yàn)分組。選擇1995-2001年因閉合性顱腦損傷死亡并伴有明顯大腦皮質(zhì)挫傷灶的35個(gè)案例,分成7個(gè)實(shí)驗(yàn)組,對(duì)照組選取非顱腦損傷且急性損傷致死5個(gè)案例,各組男女不限,年齡在19-58歲之間,平均年齡為38.09歲。 2.免疫組織化學(xué)染色方法。石蠟切片常規(guī)脫蠟至水;3%H_2O_2滅活內(nèi)源性過(guò)氧化物酶;PBS沖洗;抗原修復(fù);PBS沖洗;正常血清封閉;一抗,37℃1h;PBS沖洗;二抗,37℃30min;PBS沖洗;滴加 SP(或 SABC),37℃30min;PBS沖洗;DAB顯色后水洗;蘇木素復(fù) 染,脫水、透明、封片。 3.圖像分析與統(tǒng)計(jì)學(xué)處理。應(yīng)用MOTICAM 1300圖像分析 儀,對(duì)免疫組織化學(xué)染色呈陽(yáng)性的細(xì)胞定量檢測(cè)其灰度值,然后在 SPSS 10.0 for Window軟件下進(jìn)行統(tǒng)計(jì)學(xué)處理。 實(shí)驗(yàn)結(jié)果 *結(jié)構(gòu)型HSP70免疫組織化學(xué)染色。即刻死亡組HSpoo陽(yáng) 性神經(jīng)細(xì)胞灰度值最小,與其它各實(shí)驗(yàn)組相比有顯著差異(P< 0.01人隨挫傷時(shí)間的延長(zhǎng),神經(jīng)細(xì)胞的 HSP70灰度值逐漸增大, 至20-27h組,HSpoo灰度值達(dá)最大,與其它各實(shí)驗(yàn)組(除10.5- 11.sd組)相比有顯著差異(P<O.01人 以后各實(shí)驗(yàn)組mn0 灰度值有所下降,10.5刁1.sd組HSP70灰度值又再次升高。 2.bFGF免疫組織化學(xué)染色。即刻死亡組hFGF陽(yáng)性神經(jīng)細(xì) 胞灰度值最小,與其它各實(shí)驗(yàn)組相比有顯著差異(P<O.01人 10-14h組bFGF灰度值急劇增力(P<0.0),以后bFGF灰度 值緩慢升高,至 10.5叫.sd組,灰度值達(dá)最大。 3.COXZ免疫組織化學(xué)染色。10-1均組 COXZ陽(yáng)性神經(jīng)細(xì) 胞的灰度值開(kāi)始顯著不久p<{一n,至排一月h組COXZ灰度值 達(dá)最{ 鄉(xiāng)與其{ 各實(shí)驗(yàn)主卜以一入了顯名差異( <O.OU哆* 一*卜組 和 2.5-3.sd組陽(yáng)性全月再度值開(kāi)步 升高,6。5-7.sd組陽(yáng)性細(xì) 胞灰度值達(dá)最大,與其他各實(shí)驗(yàn)組相比有顯著差異沖<O.01廠 10.5-11.sd組未見(jiàn) COXZ 的表達(dá)。 討 論 1.HSpoo免疫組織化學(xué)染色結(jié)果分析。Seidbers等應(yīng)用 Western印跡發(fā)現(xiàn)8例腦外傷手術(shù)患者的受損腦組織的結(jié)構(gòu)型 HSP70與對(duì)照組相比沒(méi)有差異。本實(shí)驗(yàn)檢測(cè)人腦挫傷后結(jié)構(gòu)型 ·2· HSpoo的表達(dá)情況,結(jié)果顯示,各實(shí)驗(yàn)組中,即刻死亡組神經(jīng)細(xì)胞 HSP70表達(dá)最強(qiáng),HSpoo表達(dá)在損傷后 id顯著下降,以后隨著腦 挫傷時(shí)間的延長(zhǎng)而有所恢復(fù),這與Seidbers的實(shí)驗(yàn)結(jié)果存在較大 差異,原因是本實(shí)驗(yàn)案例的顱腦損傷程度較Seidbers的實(shí)驗(yàn)對(duì)象 更為嚴(yán)重,同時(shí)所采用的實(shí)驗(yàn)方法也不相同,所以,實(shí)驗(yàn)結(jié)果存在 較大的差異。因此,可以用結(jié)構(gòu)型HSP70免疫組織化學(xué)染色推斷 人腦挫傷時(shí)間。 2.bFGF免疫組織化學(xué)染色結(jié)果分析。Finklestein等發(fā)現(xiàn)大 鼠腦損傷后五周,bFGF免疫活性顯著增加,其陽(yáng)性細(xì)胞大多數(shù)類 似于反應(yīng)性星形膠質(zhì)細(xì)胞。Frantschy等檢測(cè)大鼠中樞神經(jīng)系統(tǒng) 損傷后bFGF蛋白表達(dá)情況,發(fā)現(xiàn)在損傷灶周邊區(qū)的早期反應(yīng)kh 到7d)中增加的bFGF表達(dá)大多數(shù)存在于MAC陽(yáng)性細(xì)胞(巨噬細(xì) 胞和/或膠質(zhì)細(xì)胞)中。本實(shí)驗(yàn)即刻死亡組的陽(yáng)性神經(jīng)細(xì)胞bFGF 表達(dá)最強(qiáng),傷后lid內(nèi)隨時(shí)間的增加其表達(dá)不斷下降。本實(shí)驗(yàn) bFGF表達(dá)與動(dòng)物實(shí)驗(yàn)差異的形成原因可能有以下幾個(gè)方面:①實(shí) 驗(yàn)對(duì)象的差異。②顱腦損傷程度不一致。③由于腦損傷后,挫傷 周邊區(qū)反應(yīng)性缺血、缺氧,細(xì)胞反應(yīng)性減低,從而抑制蛋白質(zhì)的合 成。因此,可以用bFGF免疫組織化學(xué)染色推斷人腦挫傷時(shí)間。 3.COXZ免疫組織化學(xué)染色結(jié)果分析。Str3SSS等人在大鼠外 傷性腦損傷后COXZ表達(dá)研究中發(fā)現(xiàn),神經(jīng)細(xì)胞COXZ免疫活性 于傷后Zh在4h和3d時(shí)升高,其中在24h顯著增加,于傷后7d返 回至對(duì)照組水平。本實(shí)驗(yàn)檢測(cè)人腦挫傷后COXZ的表達(dá)情況,結(jié) 果發(fā)現(xiàn),重度顱腦損傷后 COXZ表達(dá)開(kāi)始升高,在傷后 id左右達(dá) 峰值,以后逐步下降,其表達(dá)變化規(guī)律同Str8SSS等人的實(shí)驗(yàn)結(jié)果 基本一致。但本實(shí)驗(yàn)發(fā)現(xiàn)2.5-3.sd組和 6.5-7.sd組 COXZ表 達(dá)迅速下降,顯著低于對(duì)照組,傷后 11d,COXZ表達(dá)消失,造成此 種現(xiàn)象的
