【摘要】：研究背景及目的:在法醫(yī)學(xué)實(shí)踐中,歷來(lái)對(duì)損傷時(shí)間的推斷建立在創(chuàng)傷修復(fù)反應(yīng)的病理生理學(xué)基礎(chǔ)之上,即以不同經(jīng)歷時(shí)間的創(chuàng)口在修復(fù)過(guò)程中所表現(xiàn)的生活反應(yīng)(vital reaction)特點(diǎn)——炎癥反應(yīng)、肉芽組織及上皮增生和疤痕修復(fù)所產(chǎn)生的各種形態(tài)、組化及生化變化特征作為推斷損傷時(shí)間和性質(zhì)的依據(jù)。機(jī)體損傷大多導(dǎo)致血管內(nèi)皮的損傷,其直接后果是凝血過(guò)程的啟動(dòng)及其后的組織充血水腫,炎細(xì)胞浸潤(rùn),組織修復(fù)再生等,其間von Willebrand Factor(vWF)對(duì)損傷血管內(nèi)皮部位血小板的粘附聚集起到了重要作用;血管內(nèi)皮生長(zhǎng)因子(Vascular endothelial growth facto, VEGF)是作用最強(qiáng),特異性最高的血管生成調(diào)控因子,在損傷的早期起到調(diào)節(jié)血管內(nèi)皮細(xì)胞通透性,趨化炎細(xì)胞的作用,在修復(fù)期起改變細(xì)胞外基質(zhì),使其更易于血管的生長(zhǎng)。在創(chuàng)傷的修復(fù)過(guò)程中,細(xì)胞—生長(zhǎng)因子—細(xì)胞外基質(zhì)(EcM, extracellular matrix)三者相互作用、相互影響的復(fù)雜動(dòng)態(tài)過(guò)程,生長(zhǎng)因子參與了創(chuàng)傷愈合的整個(gè)過(guò)程并在其中發(fā)揮著舉足輕重的作用。創(chuàng)傷修復(fù)過(guò)程中,內(nèi)源性生長(zhǎng)因子起著重要的調(diào)控作用,因而檢查創(chuàng)傷后機(jī)體內(nèi)的凝血因子及生長(zhǎng)因子的改變,使更加精確地推斷損傷時(shí)間成為可能。 方法:健康Sprague-Dawley大鼠(S-D大鼠)65只,隨機(jī)分為生前傷組、死后傷組和正常對(duì)照組,每組5只:生前傷組按處死時(shí)間不同分為15min、30min、1h、3h、6h、12h、1d、3d、5d、7d共十個(gè)亞組;
[Abstract]:Background and objective: in forensic practice, the estimation of injury time has always been based on the pathophysiological basis of wound repair response. That is, the (vital reaction) characteristics of life response in the repair process of wounds over different periods of time-inflammatory reactions, granulation tissue and epithelial hyperplasia, and various forms of scar repair. Histochemical and biochemical changes were used as the basis for estimating the time and nature of injury. The direct consequence of the injury is the initiation of coagulation process and the subsequent hyperemia and edema, inflammatory cell infiltration, tissue repair and regeneration, etc. Von Willebrand Factor (vWF) plays an important role in platelet adhesion and aggregation in injured vascular endothelium. Vascular endothelial growth factor (Vascular endothelial growth facto, VEGF) is the strongest and most specific angiogenesis regulator, which regulates the permeability and chemotaxis of vascular endothelial cells in the early stage of injury. The extracellular matrix is altered during the repair phase to make it easier for blood vessels to grow. In the process of wound repair, the complex dynamic process of interaction of cell-growth factor-extracellular matrix (EcM, extracellular matrix), Growth factor is involved in the whole process of wound healing and plays an important role in it. Endogenous growth factor plays an important role in the process of wound repair, so it is possible to detect the changes of coagulation factor and growth factor in the body after trauma, so it is possible to infer the injury time more accurately. Methods: 65 healthy Sprague-Dawley rats (S-D rats) were randomly divided into antemortem injury group, postmortem injury group and normal control group with 5 rats in each group. There were ten subgroups in 7 days.
