額竇數(shù)字X線影像法醫(yī)學(xué)同一認(rèn)定的研究

【摘要】: 【研究背景及目的】應(yīng)用放射影像資料進(jìn)行同一認(rèn)定是法醫(yī)放射學(xué)重要的研究課題之一,因人類(lèi)額竇獨(dú)特多樣的形態(tài)特征使其成為顱面中最常應(yīng)用的識(shí)別標(biāo)志。采用人類(lèi)額竇放射影像學(xué)資料對(duì)個(gè)體進(jìn)行個(gè)人識(shí)別在法醫(yī)學(xué)中應(yīng)用越來(lái)越廣泛,這是基于單個(gè)人體的額竇具有高度個(gè)人特異、終生不變、難于偽造、易于觀察與測(cè)量等特征,,可以用于法醫(yī)學(xué)同一認(rèn)定。然而,從證據(jù)學(xué)角度考察,額竇同一認(rèn)定的方法與指標(biāo)還有待客觀化、標(biāo)準(zhǔn)化。本研究旨在于進(jìn)一步探究額竇的數(shù)字X線影像學(xué)分型和同一認(rèn)定的指標(biāo)及數(shù)字編碼。 【方法】(1)按照納入與排除標(biāo)準(zhǔn)收集額竇CR片(2)在額竇CR片上作出與兩側(cè)眼眶上緣均相切的直線,(3)并觀察此直線與額竇的位置關(guān)系,對(duì)額竇形態(tài)學(xué)特征進(jìn)行分型研究(4)同時(shí)篩選出額竇不對(duì)稱(chēng)性、左側(cè)額竇面積與左側(cè)眼眶面積比、右側(cè)額竇面積與右側(cè)眼眶面積比、較大額竇面積側(cè)別、左側(cè)額竇上緣弓形彎曲數(shù)、右側(cè)額竇上緣弓形彎曲數(shù)、額竇部分分隔位置、額竇中間隔位置8項(xiàng)指標(biāo)進(jìn)行描述性統(tǒng)計(jì)分析和數(shù)字編碼研究。 【結(jié)果】(1)提出額竇數(shù)字X線影像學(xué)特征六類(lèi)分型:Ⅰ標(biāo)準(zhǔn)型、Ⅱ單側(cè)缺失型、Ⅲ單側(cè)發(fā)育不全型、Ⅳ雙側(cè)發(fā)育不全型、Ⅴ無(wú)中間隔型、Ⅵ未發(fā)育型(2)篩選出額竇個(gè)人識(shí)別指標(biāo):額竇不對(duì)稱(chēng)性、較大額竇面積的側(cè)別、左右側(cè)額竇上緣弓形彎曲數(shù)、額竇部分隔位置以及額竇中間隔的位置性別差異無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05);額竇面積與眼眶面積的比值性別差異在統(tǒng)計(jì)學(xué)上有意義,且男性大于女性(P<0.05)。 【結(jié)論】(1)經(jīng)400例額竇數(shù)字X線影像分型與數(shù)字編碼研究再次證實(shí)額竇形態(tài)具有高度個(gè)人特異性,可以用于法醫(yī)學(xué)同一認(rèn)定;(2)提出額竇數(shù)字X線影像學(xué)的六類(lèi)分型:Ⅰ標(biāo)準(zhǔn)型、Ⅱ單側(cè)缺失型、Ⅲ單側(cè)發(fā)育不全型、Ⅳ雙側(cè)發(fā)育不全型、Ⅴ無(wú)中間隔型、Ⅵ未發(fā)育型;(3)額竇不對(duì)稱(chēng)性、較大額竇面積的側(cè)別、左右側(cè)額竇上緣弓形彎曲數(shù)、額竇部分隔位置以及額竇中間隔的位置7項(xiàng)指標(biāo)無(wú)性別差異,僅額竇面積與眼眶面積比值1項(xiàng)指標(biāo)有性別差異;(4)采用額竇數(shù)字X線影像進(jìn)行同一認(rèn)定的步驟:可先作類(lèi)別分型,再用所選取的8項(xiàng)指標(biāo)對(duì)額竇形態(tài)編制數(shù)字識(shí)別編碼。
[Abstract]:[background and objective] using radiographic data to identify the same is one of the most important research topics in forensic radiology. Because of the unique and diverse morphological characteristics of human frontal sinus, it has become the most commonly used identification marker in craniofacial. The personal identification of individuals using radiographic data of human frontal sinus is becoming more and more widely used in forensic medicine. This is based on the fact that the frontal sinus of a single human body has the characteristics of highly personal specificity, lifetime invariance, difficulty in forgery, easy observation and measurement, and can be used for forensic identification. However, from an evidential point of view, The objective of this study is to further explore the digital radiographic classification of frontal sinus and the index and coding of the same recognition. [methods] according to the criteria of inclusion and exclusion, the frontal sinus CR films were collected. The anatomic characteristics of frontal sinus were classified and analyzed. The asymmetry of frontal sinus was also screened out. The ratio of left frontal sinus area to left orbital area, right frontal sinus area to right orbital area, and larger frontal sinus area were found. The number of arcuate curves in the upper margin of the left frontal sinus, the number of arcuate curves in the superior margin of the right frontal sinus, the partial septal position of the frontal sinus and the middle septal position of the frontal sinus were studied by descriptive statistical analysis and numerical coding.
