類胰蛋白酶和IgE診斷過(guò)敏性休克的實(shí)驗(yàn)研究和法醫(yī)學(xué)意義

【摘要】:目的:過(guò)敏性休克在臨床醫(yī)學(xué)實(shí)踐中頗為常見(jiàn),它是由于特異性過(guò)敏原作用于致敏病人而引起的以循環(huán)衰竭為主的速發(fā)型變態(tài)反應(yīng)。它發(fā)病急劇,難以預(yù)見(jiàn),死亡率較高。極易引起醫(yī)療糾紛等一系列法律問(wèn)題,且一般常規(guī)尸檢無(wú)特異性病理變化,所以過(guò)敏性休克一直是法醫(yī)學(xué)鑒定中的一大難題。本實(shí)驗(yàn)試圖尋找診斷過(guò)敏性休克的敏感、客觀指標(biāo)。 方法:18只體重200克左右的豚鼠,隨機(jī)分為實(shí)驗(yàn)組和對(duì)照組(每組各9只)。建立多人血清致敏的豚鼠過(guò)敏性休克動(dòng)物模型,留取豚鼠血清、呼吸道及消化道組織,(1)應(yīng)用放射免疫學(xué)及酶免疫熒光方法,對(duì)血清中IgE和類胰蛋白酶水平進(jìn)行檢測(cè);(2)對(duì)呼吸道、消化道組織進(jìn)行IgE免疫組化的方法染色。 結(jié)果:試驗(yàn)組與對(duì)照組相比,(1)血清IgE水平顯著升高(實(shí)驗(yàn)組1.56±0.31IU/ML,,對(duì)照組0.64±2.53 IU/ML,p<0.01=,血清類胰蛋白酶水平顯著增高(實(shí)驗(yàn)組 1.86±0.24ng/ml,對(duì)照組0.518±0.16ng/ml,p<0.01=;(2)呼吸道及消化道粘膜IgE免疫組化染色陽(yáng)性率增高(p0.01)。 結(jié)論:與對(duì)照組比較,過(guò)敏性休克時(shí),(1)血清IgE水平及類胰蛋白酶水平增高;(2)IgE免疫組化發(fā)現(xiàn)實(shí)驗(yàn)組表達(dá)顯著增高。因此,通過(guò)放射免疫及酶免疫熒光法檢測(cè)血清IgE和類胰蛋白酶水平,并結(jié)合免疫組化法檢測(cè)咽喉、肺及胃腸組織的IgE,可以輔助診斷過(guò)敏性休克。
[Abstract]:Objective: anaphylactic shock is quite common in clinical practice. It is a rapid allergic reaction mainly caused by circulatory failure caused by specific allergen in sensitized patients. The onset of anaphylactic shock is rapid and unpredictable. The mortality rate is high. It is easy to cause a series of legal problems, such as medical disputes, and general routine autopsy has no specific pathological changes. Therefore, anaphylactic shock has always been a difficult problem in forensic identification. This experiment attempts to find sensitive and objective indicators for the diagnosis of anaphylactic shock. Methods Twenty eight guinea pigs weighing about 200 g were randomly divided into experimental group and control group (9 guinea pigs in each group). The levels of IgE and trypsin in serum were detected by radioimmunoassay and enzyme immunofluorescence. 2) IgE immunohistochemical staining was performed on the respiratory and digestive tract tissues. Results: compared with the control group, the serum IgE level in the experimental group was significantly higher than that in the control group (1.56 鹵0.31 IUU / MLand 0.64 鹵2.53 IU/ML in the control group). P < 0.01, serum trypsin level increased significantly (experimental group 1.86 鹵0.24ng / ml, control group 0.518 鹵0.16ng / ml < 0.01g / ml); 2) the positive rate of IgE immunohistochemical staining in the mucous membrane of the respiratory tract and digestive tract was higher than that in the control group (P 0.01). Conclusion: compared with the control group, the serum levels of IgE and trypsin in anaphylactic shock patients were higher than those in the control group. The expression of IgE and trypsin in serum were detected by radioimmunoassay and enzyme-immunofluorescence assay, and the pharynx and larynx were detected by immunohistochemistry. IgE in lung and gastrointestinal tissues can assist in the diagnosis of anaphylactic shock.
