【摘要】：女性盆底功能障礙性疾病是中老年女性的常見疾病,現(xiàn)已成為威脅女性健康的5種常見慢性疾病之一。盆底支撐結(jié)構(gòu)中,盆底肌肉群的支撐作用至關(guān)重要,因此盆底肌肉評估對于女性盆底功能障礙性疾病的診療具有重要的臨床價值。提出一種無創(chuàng)客觀的肌肉運動分析方法,基于經(jīng)會陰超聲檢測手段,同步獲取50例具有不同脫垂程度女性主動收縮肌肉時的超聲視頻數(shù)據(jù)及陰道內(nèi)壓數(shù)據(jù),應(yīng)用二維彈性成像算法追蹤盆底肌位移場,提取特征點的位移曲線,對比分析能有效完成指定動作的37例不同脫垂程度患者的位移結(jié)果,發(fā)現(xiàn)提取接近恥骨處特征點的切向位移參數(shù)(M_(Pu))與臨床脫垂測量參數(shù)(L_(BP))具有相關(guān)性(r=-0.93),同時通過在指定的肌肉慢縮運動中的肌力維持時間均值和最大厚度均值對盆底肌的持續(xù)控制能力進行評估,結(jié)果與被試臨床脫垂分級表現(xiàn)具有顯著相關(guān)性。結(jié)果表明,這種方法可實現(xiàn)對盆底肌肉自身生物力學特性的客觀定量評估,為臨床脫垂診斷提供重要的參考價值。
[Abstract]:Pelvic floor dysfunction is one of the five common chronic diseases that threaten women's health. Pelvic floor muscles play an important role in the supporting structure of pelvic floor. Therefore, pelvic floor muscle assessment has important clinical value for the diagnosis and treatment of female pelvic floor dysfunction disease. A noninvasive and objective method of muscle motion analysis based on transperineal ultrasound was proposed. The ultrasonic video data and intravaginal pressure data of 50 women with different prolapse degrees were obtained simultaneously. The displacement field of pelvic floor muscle was tracked by two-dimensional elastic imaging algorithm and the displacement curve of characteristic points was extracted. The results of displacement in 37 patients with different prolapse degrees were compared and analyzed. It was found that there was a correlation between the extraction of tangential displacement parameters close to the characteristic point of pubis and the clinical prolapse measurement parameter (LSP). At the same time, the duration control ability of pelvic floor muscle was evaluated by the mean of muscle force maintenance time and maximum thickness in the specified muscle slow contraction. The results showed that this method could be used to evaluate the biomechanical characteristics of pelvic floor muscles. It provides important reference value for clinical prolapse diagnosis.
