【摘要】：目的觀察集束化護(hù)理策略聯(lián)合中藥腸道去污療法在嚴(yán)重骨創(chuàng)傷后急性呼吸窘迫綜合癥(ARDS)患者臨床治療中的應(yīng)用及療效評(píng)價(jià)。方法 150例患者隨機(jī)平均分入A組、B組、C組。A組采用西醫(yī)常規(guī)護(hù)理及治療,B組采用集束化護(hù)理聯(lián)合西醫(yī)常規(guī)治療,C組在B組治療方案上加用中藥腸道去污療法。觀察3組患者治療前及治療后7d吸氧濃度(FiO_2)、動(dòng)脈氧分壓(PaO_2)、氧和指數(shù)(PaO_2/FiO_2)、潮氣量(Vt)的變化,呼吸機(jī)相關(guān)性肺炎(VAP)的發(fā)生率,ICU總費(fèi)用、入住和使用呼吸機(jī)時(shí)間,口腔、PICC、中心靜脈置管細(xì)菌培養(yǎng)陽性率,以及60日死亡率和壓瘡發(fā)生率、非計(jì)劃拔管率、誤吸發(fā)生率。結(jié)果治療后7d,C組FiO_2、PaO_2、PaO_2/FiO_2、Vt較A、B兩組明顯改善,B組與A組對(duì)比有統(tǒng)計(jì)學(xué)意義(P0.05)。B、C兩組于VAP發(fā)生率、各細(xì)菌學(xué)培養(yǎng)陽性率、60日死亡率比較中無明顯差異(P0.05),與A組比較有統(tǒng)計(jì)學(xué)意義(P0.05)。C組ICU總費(fèi)用、入住和使用呼吸機(jī)時(shí)間較A、B兩組存在統(tǒng)計(jì)學(xué)意義(P0.05),B組與A組對(duì)比有明顯差異(P0.05)。結(jié)論集束化護(hù)理策略聯(lián)合中藥腸道去污與西醫(yī)常規(guī)治療嚴(yán)重骨創(chuàng)傷后ARDS,能有效改善患者通氣功能,降低VAP發(fā)生率、死亡率以及病菌感染風(fēng)險(xiǎn),縮短ICU入住及呼吸機(jī)使用時(shí)間,較常規(guī)治療護(hù)理更經(jīng)濟(jì),操作性強(qiáng),依從性更佳。
[Abstract]:Objective to observe the clinical application and evaluation of cluster nursing strategy combined with traditional Chinese medicine for intestinal decontamination in patients with acute respiratory distress syndrome (ARDS) after severe bone trauma. Methods 150 patients were randomly divided into group A, group B and group C. Group A was treated with routine western medicine, group B was treated with cluster nursing combined with routine western medicine, group C was treated with traditional Chinese medicine for intestinal decontamination. The changes of oxygen concentration (FiO_2), arterial partial pressure of oxygen (PaO_2), oxygen and index (PaO_2/FiO_2), tidal volume (Vt) and the incidence of ventilator-associated pneumonia (VAP) were observed before and 7 days after treatment. The total cost of ICU, the time of admission and use of ventilator, the positive rate of bacterial culture in oral cavity and central venous tube of PICC, the death rate of 60 days and the incidence of pressure sore, unplanned extubation rate and missuction rate. Results 7 days after treatment, FiO_2,PaO_2,PaO_2/FiO_2,Vt in group C was significantly improved than that in group A (P 0.05). The incidence of VAP in group B was significantly higher than that in group A (P0.05), and the positive rate of bacterial culture in group B was higher than that in group A (P 0.05). There was no significant difference in 60-day mortality (P0.05), but there was significant difference between group A and group A (P0.05). C group were significantly higher than those in group A (P0.05), and there was no significant difference between group A and group A (P0.05). There was significant difference between group B and group A (P0.05). Conclusion the cluster nursing strategy combined with traditional Chinese medicine for intestinal decontamination and routine western medicine in the treatment of ARDS, after severe bone trauma can effectively improve the ventilation function of patients, reduce the incidence of VAP, mortality and the risk of bacterial infection, shorten the duration of ICU stay and ventilator use. Compared with conventional treatment and nursing, it is more economical, more operable and more compliant.
