【摘要】：正隨著醫(yī)療水平的提升及優(yōu)質(zhì)護(hù)理的深入開展,患者對(duì)醫(yī)療服務(wù)的要求越來越高,傳統(tǒng)醫(yī)療照護(hù)服務(wù)無法滿足患者出院后的護(hù)理需求,患者在出院后仍會(huì)遇到不同的健康問題~([1-2])。在全世界致力于改善患者出院后生活質(zhì)量的大背景下~([3]),患者出院后護(hù)理的協(xié)調(diào)性和連續(xù)性問題逐漸受到重視。而延續(xù)護(hù)理對(duì)于改善患者預(yù)后、促進(jìn)患者康復(fù)、提高患者生活質(zhì)量、降低再住院率及衛(wèi)生服務(wù)成本均具有十分重要的意義~([4-5])。所謂延續(xù)護(hù)理是指通過一系列的行動(dòng)
[Abstract]:With the improvement of medical level and the in-depth development of quality nursing, patients are demanding more and more medical services. The traditional medical care services can not meet the nursing needs of patients after discharge. Patients still encounter different health problems after discharge. In the context of worldwide efforts to improve the quality of life of patients after discharge, the coordination and continuity of nursing care after discharge has gradually been paid attention to. Continuous nursing is of great significance in improving the prognosis of patients, promoting the recovery of patients, improving the quality of life of patients, reducing the rate of rehospitalization and the cost of health services.
