【摘要】：目的:研究本院耐碳青霉烯類肺炎克雷伯菌(CRKP)耐藥性以及菌株間的同源性。方法:全自動微生物分析儀進行細(xì)菌鑒定及藥敏試驗,采用改良Hodge試驗和EDTA協(xié)同試驗檢測耐藥表型,PCR檢測碳青霉烯酶基因,脈沖場凝膠電泳(PFGE)分析菌株同源性。結(jié)果:26株肺炎克雷伯菌對頭孢菌素類、單環(huán)β-內(nèi)酰胺類、氨基糖苷類、喹喏酮類、呋喃妥因等多種抗菌藥物耐藥;經(jīng)PCR檢測后25株攜帶KPC基因,1株攜帶IMP基因,未檢測出VIM、NDM-1、OXA-48基因;PFGE電泳結(jié)果顯示,26株肺炎克雷伯菌分為4型。其中Ⅰ、Ⅱ為主要流行克隆型。結(jié)論:本院存在肺炎克雷伯菌耐藥株的傳播,攜帶的耐藥基因以KPC-2為主,主要以ICU與MICU的2株耐藥株流行,而且耐藥菌株間存在著不同程度的親緣關(guān)系。
[Abstract]:Objective: to study the (CRKP) resistance of Klebsiella pneumoniae resistant to carbapenem and its homology. Methods: the bacterial identification and drug sensitivity test were carried out by automatic microbiological analyzer. The phenotype of drug resistance was detected by modified Hodge test and EDTA co-assay. The carbapenase gene was detected by PCR and the homology of the strains was analyzed by pulsed field gel electrophoresis (PFGE). Results: 26 strains of Klebsiella pneumoniae were resistant to cephalosporins, monocyclic 尾 -lactams, aminoglycosides, quinolones and furantoin. After PCR detection, 25 strains carried KPC gene, 1 strain carried IMP gene, and no VIM,NDM-1,OXA-48 gene was detected.
