【摘要】：目的探討非糖尿病腹膜透析患者胰島素抵抗與動脈粥樣硬化的關(guān)系。方法選擇非糖尿病腹膜透析的成年患者64例,對其臨床資料包括胰島素抵抗指數(shù)(HOMA-IR)進(jìn)行分析。采用高分辨二維超聲對雙側(cè)頸總動脈內(nèi)膜中膜厚度(IMT)及頸動脈粥樣斑塊進(jìn)行測量。按IMT厚度及有無斑塊分為兩組:無頸動脈硬化組(IMT1.0 mm)和頸動脈硬化組(IMT≥1.0 mm和/或已發(fā)現(xiàn)有頸動脈斑塊)。結(jié)果頸動脈硬化組餐后2 h血糖(2h PBG)、低密度脂蛋白膽固醇(LDLC)、高敏C反應(yīng)蛋白(hs-CRP)、甘油三酯(TG)和HOMA-IR高于無頸動脈硬化組(P0.05),而血漿白蛋白則低于無頸動脈硬化組(P0.01);頸動脈硬化組胰島素抵抗患者占53.49%,而無頸動脈硬化組胰島素抵抗患者僅占4.76%(P0.01)。相關(guān)分析顯示非糖尿病腹膜透析患者動脈硬化與年齡、2h PBG、LDLC、hs-CRP、TG和HOMA-IR呈正相關(guān)(r=0.375、0.373、0.420、0.345、0.351、0.458,P0.05);Logistic回歸分析發(fā)現(xiàn),年齡、HOMA-IR、2h PBG、hs-CRP是非糖尿病腹膜透析患者發(fā)生動脈粥樣硬化的危險因素。結(jié)論在非糖尿病腹膜透析患者中HOMA-IR水平與頸動脈粥樣硬化正相關(guān),HOMA-IR可能促進(jìn)了動脈粥樣硬化的發(fā)生。
[Abstract]:Objective to investigate the relationship between insulin resistance and atherosclerosis in non-diabetic peritoneal dialysis patients. Methods 64 adult patients with non-diabetic peritoneal dialysis were selected and their clinical data including insulin resistance index (HOMA-IR) were analyzed. The intima media thickness (IMT) and carotid atherosclerotic plaque of bilateral common carotid artery were measured by two dimensional high resolution ultrasound.
