【摘要】：目的研究廣東省人民醫(yī)院真菌性血流感染患者的病原學(xué)及臨床特點(diǎn),進(jìn)一步探討真菌性血流感染的防治措施。方法回顧性分析醫(yī)院250例真菌性血流感染患者的臨床資料,描述其基礎(chǔ)疾病、危險因素、轉(zhuǎn)歸等,并采用χ2檢驗(yàn),進(jìn)行預(yù)后單因素分析,采用logistic回歸進(jìn)行預(yù)后多因素分析。結(jié)果 250例患者血培養(yǎng)共分離出真菌261株,真菌中最常見的是念珠菌屬234株占89.7%;住院期間因各種原因死亡87例,粗病死率為34.8%;其中因真菌性血流感染死亡50例,歸因病死率為20.0%;單因素分析顯示中心靜脈置管、機(jī)械通氣、留置尿管、留置引流管、ICU治療、使用糖皮質(zhì)激素、年齡≥60與住院期間死亡相關(guān);多因素分析中,氣管插管、年齡≥60、使用糖皮質(zhì)激素增加真菌性血流感染患者住院期間死亡風(fēng)險。結(jié)論真菌性血流感染病原菌以念珠菌屬為主,尤其是白色念珠菌,出現(xiàn)真菌性血流感染的患者預(yù)后差,氣管插管、年齡≥60、使用糖皮質(zhì)激素等增加真菌性血流感染患者住院期間死亡風(fēng)險。
[Abstract]:Objective to study the etiology and clinical features of fungal blood stream infection in Guangdong Provincial people's Hospital. Methods the clinical data of 250 patients with fungal blood flow infection in hospital were analyzed retrospectively. The underlying diseases, risk factors and prognosis were described. The results were analyzed by 蠂 2 test, single factor analysis of prognosis and multivariate analysis of prognosis by logistic regression. Results 261 strains of fungi were isolated from blood culture of 250 patients, the most common fungi were 234 strains of Candida, 87 cases died from various causes, the crude mortality rate was 34.8%, and 50 cases died of fungal blood stream infection.
