【摘要】：目的探討留置針穿刺角度與穿刺成功率之間的關(guān)系。方法選擇直徑分別為2mm和3mm的模擬血管,選取15°、20°、25°、30°、35°、40°、45°作為穿刺角度,運(yùn)用數(shù)學(xué)的方法,計(jì)算出針尖在血管內(nèi)的可用距離,并繪制出對(duì)模擬血管進(jìn)行不同穿刺角度穿刺的圖形,計(jì)算出的數(shù)字用圖表的的方式表示。選取2013年7月~2014年12月我院耳鼻喉科住院病人100例,術(shù)前隨機(jī)分為兩組,觀察組50例,留置針穿刺角度選擇20°進(jìn)針,對(duì)照組50例,選擇30°進(jìn)針。結(jié)果無(wú)論選擇2mm還是3mm的模擬血管,穿刺角度分別為15°、20°、25°、30°、35°、40°、45°時(shí),所產(chǎn)生的可用距離的數(shù)字顯示,隨著穿刺角度的增加,可用距離逐漸減少。統(tǒng)計(jì)100例留置針穿刺的成功率,觀察組成功率為98%,對(duì)照組為84%,觀察組顯著高于對(duì)照組(χ2=5.983,P=0.014)。結(jié)論留置針穿刺角度越小,可用距離越長(zhǎng),穿刺成功率越高,穿刺角度為15~20°時(shí)可用距離最長(zhǎng)。臨床實(shí)踐中選擇20°作為穿刺角度,不但穿刺的成功率高,而且病人的疼痛也較輕。
[Abstract]:Objective to explore the relationship between the angle of indwelling needle puncture and the success rate of puncture. Methods the simulated blood vessels with the diameter of 2mm and 3mm were selected, and the puncture angle was 15 擄20 擄20 擄25 擄30 擄30 擄30 擄30 擄35 擄40 擄40 擄45 擄. By using mathematical method, the usable distance between the needle tip and the blood vessel was calculated, and the puncture pattern of different puncture angles for the simulated blood vessel was drawn. The calculated number is represented by a chart. From July 2013 to December 2014, 100 inpatients with otolaryngology in our hospital were randomly divided into two groups: the observation group (n = 50) and the control group (n = 50). Results when the puncture angles of 2mm and 3mm were 15 擄, 20 擄, 25 擄, 30 擄, 35 擄and 40 擄40 擄45 擄, the available distance decreased gradually with the increase of puncture angle. The success rate of indwelling needle puncture in 100 cases was counted. The observed composition power was 98 and 84 in the control group, which was significantly higher in the observation group than in the control group (蠂 ~ 2 = 5.983). Conclusion the smaller the angle of indwelling needle is, the longer the usable distance is, and the higher the successful rate of puncture is, the longest available distance is when the puncture angle is 1520 擄. Choosing 20 擄as puncture angle in clinical practice not only has high success rate, but also has mild pain.
