【摘要】：目的:分析我院化膿性腦膜炎病原菌的分布及耐藥特點,為抗菌藥物的合理使用提供參考。方法:對我院2011年1月1日-2014年12月31日送檢的4 255例化膿性腦膜炎住院患者腦脊液病原菌進行培養(yǎng)、鑒定和藥敏試驗。結(jié)果:共分離出834株病原菌,檢出陽性率為19.6%,包括革蘭氏陽性菌576株,占69.1%;革蘭氏陰性菌255株,占30.6%;真菌3株,占0.4%。革蘭氏陽性菌排前3位的是凝固酶陰性葡萄球菌(436株)、金黃色葡萄球菌(56株)和腸球菌屬(29株);革蘭氏陰性菌排前3位的是肺炎克雷伯菌(46株)、鮑曼不動桿菌(38株)和大腸埃希菌(31株)。病原菌分布科室前3位是神經(jīng)外科(506株)、重癥監(jiān)護病房(169株)和呼吸重癥科(64株)。藥敏試驗結(jié)果顯示,葡萄球菌和腸球菌屬對萬古霉素、替考拉寧和米諾環(huán)素未發(fā)現(xiàn)耐藥菌株;非發(fā)酵革蘭氏陰性桿菌對米諾環(huán)素耐藥率較低。結(jié)論:我院化膿性腦膜炎患者腦脊液細菌感染病原菌檢出率以革蘭氏陽性菌為主,各種致病菌耐藥狀況嚴重,臨床應(yīng)根據(jù)藥敏情況合理選擇抗菌藥物。
[Abstract]:Objective: to analyze the distribution and drug resistance of pathogenic bacteria in suppurative meningitis in our hospital. Methods: the pathogens of cerebrospinal fluid (CSF) from 4 255 patients with suppurative meningitis from January 1, 2011 to December 31, 2014 in our hospital were cultured, identified and tested. Results: a total of 834 strains of pathogenic bacteria were isolated and the positive rate was 19.6, including 576 Gram-positive bacteria (69.1%), 255 Gram-negative bacteria (30.6%) and 3 fungi (0.445%). The top 3 Gram-positive bacteria were coagulase negative Staphylococcus (436), Staphylococcus aureus (56) and Enterococcus (29), and Gram-negative bacteria were Klebsiella pneumoniae (46) and Acinetobacter baumannii (38). E. coli (31 strains). The top 3 pathogens were neurosurgery (506), intensive care unit (169) and respiratory intensive care (64). The results of drug sensitivity test showed that Staphylococcus and Enterococcus had no resistance to vancomycin, teicoplanin and minocycline, while non-fermentative gram-negative bacilli had lower resistance to minocycline. Conclusion: the detection rate of bacterial infection in cerebrospinal fluid of patients with suppurative meningitis in our hospital is mainly Gram-positive bacteria.
