運(yùn)用延續(xù)性護(hù)理干預(yù)提高冠心病介入術(shù)后患者自我管理水平的研究

【摘要】:【目的】運(yùn)用延續(xù)性護(hù)理干預(yù)提高冠心病介入術(shù)后患者的疾病認(rèn)知和自我管理水平,改善患者預(yù)后,探索慢性心血管疾病的管理經(jīng)驗(yàn)!痉椒ā窟x取安徽省某三甲醫(yī)院心血管內(nèi)科2014年6月至2015年2月行冠狀動(dòng)脈介入治療的冠心病患者77例,采用隨機(jī)數(shù)字法分為對(duì)照組38例和試驗(yàn)組39例。對(duì)照組給予常規(guī)護(hù)理教育方法,試驗(yàn)組在常規(guī)護(hù)理教育方法的基礎(chǔ)上采用延續(xù)性護(hù)理干預(yù)(包括住院期間個(gè)體化健康教育+出院后定期護(hù)理隨訪(fǎng)),觀察比較兩組患者的疾病認(rèn)知情況和自我管理水平!窘Y(jié)果】?jī)山M患者的社會(huì)學(xué)和流行病學(xué)資料及兩組患者入院時(shí)疾病認(rèn)知和自我管理能力評(píng)分,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);試驗(yàn)組和對(duì)照組患者入院時(shí)的疾病認(rèn)知分別為7.6±6.1,7.6±4.7,得分均較低;對(duì)照組和試驗(yàn)組患者入院時(shí)的自我管理行為量表總分分別為64.6±18.3分和64.2±15.4分,得分均較低;出院時(shí)、出院3個(gè)月以及出院6個(gè)月試驗(yàn)組患者疾病認(rèn)知得分均高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。延續(xù)性護(hù)理干預(yù)能提高冠心病患者的疾病認(rèn)知水平;出院3個(gè)月、6個(gè)月試驗(yàn)組自我管理行為得分均高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);從兩組患者自我管理行為得分時(shí)間趨勢(shì)圖上,試驗(yàn)組在出院時(shí)、出院3個(gè)月以及出院6個(gè)月的自我管理行為得分逐漸上升,上升趨勢(shì)與對(duì)照組相比較明顯。延續(xù)性護(hù)理干預(yù)能提高冠心病患者的自我管理水平!窘Y(jié)論】冠心病介入術(shù)后患者疾病認(rèn)知和自我管理水平低下,需引起醫(yī)護(hù)人員重視;延續(xù)性護(hù)理干預(yù)能提高患者疾病認(rèn)識(shí)和自我管理水平。
[Abstract]:[objective] to improve the disease cognition and self-management of patients after coronary intervention by continuous nursing intervention, and to improve the prognosis of patients. To explore the management experience of chronic cardiovascular disease. [methods] Seventy-seven patients with coronary artery disease treated with coronary intervention from June 2014 to February 2015 were selected from the Department of Cardiovascular Medicine of a third Class A Hospital in Anhui Province. The patients were randomly divided into control group (n = 38) and experimental group (n = 39). The control group was given routine nursing education. On the basis of routine nursing education methods, the trial group adopted continuous nursing intervention (including regular nursing follow-up after individual health education during hospitalization), and observed and compared the disease cognition and self-management between the two groups. Level. [results] the sociological and epidemiological data of the two groups and the scores of disease cognition and self-management at admission, There was no significant difference between the two groups (P0.05), the disease cognition of the patients in the test group and the control group were 7.6 鹵6.1 鹵7.6 鹵4.7, respectively, and the total scores of self-management behavior scale were 64.6 鹵18.3 and 64.2 鹵15.4, respectively, in the control group and the experimental group. The scores of disease cognition in the trial group were higher than those in the control group at the time of discharge, 3 months after discharge and 6 months after discharge, and the difference was statistically significant (P0.05). Continuous nursing intervention could improve the disease cognition of patients with coronary heart disease, and the scores of self-management behavior in the trial group were higher than those in the control group at 3 months and 6 months after discharge. The difference was statistically significant (P0.05). According to the time trend of self-management behavior score of the two groups, the scores of self-management behavior in the experimental group increased gradually at discharge time, 3 months after discharge and 6 months after discharge, and the rising trend was more obvious than that in the control group. Continuous nursing intervention can improve the level of self-management of patients with coronary heart disease. [conclusion] patients' disease cognition and self-management level are low after coronary intervention, which should be paid more attention to by medical staff. Continuous nursing intervention can improve patients' disease awareness and self-management.
