【摘要】：【目的】通過(guò)分析肝細(xì)胞癌(HCC)MRI的主要征象與不同組織學(xué)分化程度之間的關(guān)系,探討MRI在評(píng)價(jià)肝細(xì)胞癌組織學(xué)分化程度中的價(jià)值�！痉椒ā繉⒉±碜C實(shí)的229例HCC分為3組:高分化組(34例),中分化組(170例),低分化組(25例)。均行常規(guī)MRI及DWI檢查,分析HCC瘤灶主要征象與HCC組織學(xué)分化程度的關(guān)系�！窘Y(jié)果】229例HCC中(1)HCC瘤灶的分葉程度與組織學(xué)分化程度之間呈較弱的負(fù)相關(guān)關(guān)系(r=-0.37,P=0.000);HCC的T1WI信號(hào)強(qiáng)度與組織學(xué)分化程度之間呈較弱的正相關(guān)關(guān)系(r=0.292,P=0.000);HCC的T2WI信號(hào)強(qiáng)度與組織學(xué)分化程度之間呈較弱的負(fù)相關(guān)關(guān)系(r=-0.207,P=0.002)。HCC的DWI目測(cè)信號(hào)強(qiáng)度與組織學(xué)分化程度之間呈較弱的負(fù)相關(guān)關(guān)系(r=-0.317,P=0.000)。(2)HCC的"包膜"完整性,血供方式與組織學(xué)分化程度之間無(wú)顯著相關(guān)關(guān)系(P0.05)。(3)在HCC瘤灶的分葉程度、T1WI信號(hào)強(qiáng)度、T2WI信號(hào)強(qiáng)度和DWI目測(cè)信號(hào)強(qiáng)度中,HCC瘤灶的分葉程度是影響組織學(xué)分化程度的最顯著因素(OR1=0.15,OR2=0.25,P0.05)。【結(jié)論】MRI中HCC的分葉程度、T1WI、T2WI、DWI的目測(cè)信號(hào)強(qiáng)度有利于預(yù)測(cè)HCC組織學(xué)分化程度,其中HCC分葉程度為最主要的征象;然而,根據(jù)常規(guī)MRI檢查及DWI的目測(cè)信號(hào)強(qiáng)度,而正確預(yù)測(cè)HCC的組織學(xué)分化程度是有限的,應(yīng)該綜合多種征象分析。
[Abstract]:[objective] to analyze the relationship between MRI features and histological differentiation of hepatocellular carcinoma (HCC). Methods 229 cases of HCC confirmed by pathology were divided into 3 groups: high differentiation group (34 cases), moderate differentiation group (170 cases) and low differentiation group (25 cases). All patients were examined by conventional MRI and DWI. [results] in 229 cases of HCC, there was a weak negative correlation between the degree of lobulation and the degree of histological differentiation (r-0.37 P0. 000). [results] in 229 cases of HCC, there was a weak negative correlation between the signal intensity on T 1WI and histological differentiation. There was a weak positive correlation between the degree of T2WI and the degree of histological differentiation. There was a weak negative correlation between the signal intensity of T2WI and the degree of histological differentiation (r-0.207 P0. 002). There was a weak negative correlation between the visual signal intensity of HCC and the degree of histological differentiation (r-0.317P0.000). (2). There was no significant correlation between blood supply pattern and histological differentiation (P0.05). The most important factor affecting histological differentiation was the signal intensity of T1WI and visual signal intensity of T2WI. [conclusion] the lobular degree of HCC in MRI and the visual signal intensity of T1WIT T2WI DWI are helpful to predict the degree of histological differentiation of HCC. However, according to conventional MRI and DWI visual signal intensity, it is limited to correctly predict the degree of histological differentiation of HCC.
