【摘要】：目的探討品管圈手法在降低胃腸外科患者失禁性皮炎(incontinence associated dermatitis,IAD)發(fā)生率的應(yīng)用效果,減少患者痛苦及住院時(shí)間,提高患者滿意度。方法蚌埠醫(yī)學(xué)院第一附屬醫(yī)院胃腸外科從2014年1—10月成立失禁性皮炎品管圈小組,確定以"降低胃腸外科患者失禁性皮炎發(fā)生率"為活動(dòng)主題,通過(guò)制定查檢表,1~3個(gè)月查檢患者120例應(yīng)用腦力激蕩方法查檢影響失禁性皮炎發(fā)生的因素,應(yīng)用魚骨圖、柏拉圖、柱狀圖等分析患者、家屬、醫(yī)護(hù)人員等因素對(duì)失禁性皮炎的影響,學(xué)習(xí)相關(guān)理論進(jìn)行皮膚評(píng)估,改進(jìn)護(hù)理工具,對(duì)護(hù)理流程進(jìn)行培訓(xùn),要因進(jìn)行匯總,制定真因查檢表查檢真因后確定對(duì)策實(shí)施與檢討,最后制定標(biāo)準(zhǔn)化流程在全院進(jìn)行推廣。結(jié)果本小組解決了在實(shí)施過(guò)程中遇到的壓瘡高危,更換尿墊不及時(shí),局部皮膚潮濕,護(hù)理評(píng)估不準(zhǔn)確、不及時(shí)的問(wèn)題的應(yīng)用現(xiàn)狀。結(jié)論品管圈活動(dòng)在提高個(gè)人素質(zhì)修養(yǎng)、解決問(wèn)題的能力、個(gè)人責(zé)任心、積極性、自信心、團(tuán)隊(duì)凝聚力及與患者的溝通協(xié)調(diào)能力等方面都有所提升,使護(hù)理質(zhì)量得到持續(xù)改進(jìn),其發(fā)展趨勢(shì)是廣泛應(yīng)用品管圈這個(gè)平臺(tái)來(lái)完善護(hù)理措施、細(xì)化工作流程、提高優(yōu)質(zhì)護(hù)理服務(wù)質(zhì)量。
[Abstract]:Objective to investigate the effect of quality control loop manipulation in reducing the incidence of incontinence associated dermatosis (IAD) in patients with gastrointestinal surgery, and to reduce the pain and hospitalization time of patients, and to improve the satisfaction of patients. Methods the first affiliated Hospital of Bengbu Medical College established an incontinence dermatitis quality control group from January to October 2014. The theme was "reducing the incidence of incontinence dermatitis in patients with gastrointestinal surgery". 120 patients with incontinence dermatitis were examined by using brainstorming method. The patients and their families were analyzed by fish bone map, Plato, histogram and so on. The effect of medical and nursing personnel on incontinence dermatitis, learning relevant theories to evaluate skin, improve nursing tools, train nursing process, make a table of true cause check and check true cause, determine the implementation and review of countermeasures, Finally, the standardization process is popularized in the whole hospital. Results this group solved the problems of high risk of pressure sore, not timely replacement of urine pad, local wet skin, inaccurate nursing evaluation and untimely application. Conclusion quality control activities can improve personal quality, problem-solving ability, personal responsibility, enthusiasm, self-confidence, team cohesion and ability of communication and coordination with patients, so that the quality of nursing has been continuously improved. The development trend is to apply quality control circle as a platform to perfect nursing measures, refine work flow and improve quality nursing service quality.
