【摘要】：目的探討累積平均動(dòng)脈壓(cumMAP)對(duì)全因死亡的預(yù)測(cè)價(jià)值。方法選擇參加2006-2007、2008-2009及2010-2011年度健康體檢且資料完整的開灤集團(tuán)在職及離退休職工56 039人作為觀察對(duì)象,進(jìn)行了平均4.92年的隨訪,每年通過開灤社會(huì)保障信息系統(tǒng)獲取死亡信息。cumMAP計(jì)算公式為[(MAP_1+MAP_2)/2×time_(1-2)]+[(MAP_2+MAP_3)/2×time_(2-3)],其中MAP_1、MAP_2、MAP_3分別為根據(jù)第1、2、3次體檢血壓值計(jì)算的平均動(dòng)脈血壓,time1-2、time2-3為相鄰兩次體檢血壓測(cè)量的時(shí)間間隔。將研究對(duì)象按cumMAP四分位分成4組,比較各組全因死亡率的差異,并采用多因素Cox比例風(fēng)險(xiǎn)模型分析cumMAP與全因死亡的關(guān)系。結(jié)果在平均4.92年隨訪期內(nèi)共有1805例(3.2%)發(fā)生全因死亡。cumMAP第1~4四分位組全因死亡率分別為1.1%、2.3%、3.8%、5.6%(P0.01)。多因素Cox回歸分析結(jié)果顯示,在校正包括基線收縮壓、平均動(dòng)脈壓在內(nèi)的多種混雜因素后,與第1四分位組相比,第2、3、4四分位組發(fā)生全因死亡的HR值(95%CI)分別為1.17(0.95~1.43)、1.32(1.08~1.61)和1.34(1.09~1.65)。結(jié)論 cumMAP是全因死亡的獨(dú)立危險(xiǎn)因素。
[Abstract]:Objective to investigate the predictive value of cumMAPs for total death. Methods 56,039 workers of Kailuan Group who participated in the physical examination in 2006-2007, 2008-2009 and 2010-2011 and had complete data were selected as the observation objects. The average follow-up was 4.92 years. Death information was obtained from Kailuan Social Security Information system (Kailuan Social Security Information system). The formula of CumMAP was [MPA1 MAP_2)/2 脳 MAP_2)/2 _ (1-2)] [MP-MAP2 MAP_3)/2 脳 MAP_3)/2 _ (2 ~ 3)], where MAP1MAP2MAP2MAP3 is the mean arterial blood pressure (BP) time 1 ~ 2 ~ 2 ~ 3 calculated according to the blood pressure value of the first 2 ~ (th) physical examination, respectively, which is the time interval between the two adjacent physical examination blood pressure measurements. The subjects were divided into four groups according to cumMAP quartile. The differences of all-cause mortality were compared among each group. The multivariate Cox proportional risk model was used to analyze the relationship between cumMAP and all-cause death. Results during the mean follow-up period of 4.92 years, there were 1805 cases of total death. The total death rate of the 4th quartile group of CumMAP was 1.1 and 2.3%, respectively. The results of multivariate Cox regression analysis showed that after adjusting for various confounding factors including baseline systolic blood pressure and mean arterial pressure, compared with the 1st quartile group, the HR of the 2nd 3rd and 4th quartile group were 1.170.95 and 1.431.321.321.321.081.61and 1.340.09 / 1.65g, respectively. Conclusion cumMAP is an independent risk factor for all death.
