【摘要】：目的:比較細胞DNA定量分析(DNA image cytometry,DNA-ICM)和傳統(tǒng)制片技術(shù)(Conventional Smears,CS)在頸部淋巴結(jié)和漿膜腔積液中的診斷價值。方法:利用細胞DNA定量分析技術(shù)和傳統(tǒng)制片技術(shù)對209例頸部淋巴結(jié)穿刺標本和341例漿膜腔積液標本進行比較分析。結(jié)果:209例淋巴結(jié)穿刺標本中,CS的敏感度、特異度和準確度分別為90%、98.4%及95.2%,而DNA-ICM敏感度、特異度和準確度分別為95%、100%及98.1%。統(tǒng)計學顯示兩種方法的敏感度、特異度和準確度均無顯著差異(P0.05);341例漿膜腔積液標本中,CS的敏感度、特異度和準確度分別為82.8%、93.7%及83.0%,DNA-ICM的敏感度、特異度和準確度分別為88.1%、100%及94.7%,統(tǒng)計學顯示兩種方法的敏感度無顯著差異,而特異度和準確度均有顯著差異,可見DNA-ICM在漿膜腔積液的診斷中優(yōu)于CS,且兩種方法聯(lián)合診斷可以提高診斷的敏感度。結(jié)論:DNA-ICM在淋巴結(jié)針吸標本的診斷中不優(yōu)于傳統(tǒng)制片技術(shù),并不能提高診斷的敏感度、準確度及特異度;而在漿膜腔積液的診斷中,DNA-ICM有明顯優(yōu)勢,診斷的準確度及特異度均高于傳統(tǒng)制片技術(shù)。
[Abstract]:Objective: to compare the diagnostic value of cellular DNA quantitative analysis of image cystometry DNA-ICM with conventional technique in cervical lymph node and serous cavity effusion. Methods: two hundred and nine cervical lymph node puncture specimens and 341 serous cavity effusion specimens were analyzed by cell DNA quantitative analysis and conventional film making techniques. Results the sensitivity, specificity and accuracy of CS were 9098.4% and 95.22.The sensitivity, specificity and accuracy of DNA-ICM were 95%, 100% and 98.1%, respectively. There was no significant difference in sensitivity, specificity and accuracy between the two methods. The sensitivity, specificity and accuracy of CS in 341 cases of serous effusion were 82.8% and 83.0%, respectively. The specificity and accuracy were 88. 1% and 94. 7%, respectively. There was no significant difference in sensitivity between the two methods, but there were significant differences in specificity and accuracy between the two methods. Therefore, DNA-ICM is superior to CSS in the diagnosis of serous effusion, and the combination of two methods can improve the sensitivity of diagnosis. Conclusion in the diagnosis of lymph node aspiration specimens, the sensitivity, accuracy and specificity of the diagnosis can not be improved by using DNA ICM, but it has obvious advantages in the diagnosis of serous cavity effusion, and it can not improve the sensitivity, accuracy and specificity of the diagnosis of lymph node aspiration specimen, but it has obvious advantages in the diagnosis of serous cavity effusion. The accuracy and specificity of diagnosis were higher than that of traditional technique.
