【摘要】：目的了解孕前夫妻雙方乙型肝炎病毒(HBV)、梅毒(TP)、人類免疫缺陷病毒(HIV)的感染狀況以及丙氨酸氨基轉(zhuǎn)移酶(ALT)異常率,以便采取有效措施防止夫妻間的相互傳播和母嬰垂直傳播,保障夫妻雙方和嬰兒健康。方法回顧性選取2012—2014年在河北省婦幼保健中心進(jìn)行孕前優(yōu)生健康檢查且符合納入標(biāo)準(zhǔn)的夫妻4 073對為研究對象。收集患者HBV、TP、HIV、ALT檢測結(jié)果并進(jìn)行分析。結(jié)果 8 146例研究對象中,乙型肝炎病毒表面抗原(HBs Ag)陽性檢出率為3.44%(280/8 146),TP快速血漿反應(yīng)素環(huán)狀卡片試驗(RPR)和TP螺旋體顆粒凝集試驗(TPPA)均陽性檢出率為0.34%(28/8 146),抗HIV1/2陽性檢出率為0,ALT異常檢出率為4.82%(393/8 146)。男性HBs Ag陽性、ALT異常檢出率大于女性(P0.05)。168例HBs Ag陽性男性中,HBs Ag、乙型肝炎病毒e抗體(HBe Ab)、乙型肝炎病毒核心抗體(HBc Ab)均陽性115例,HBs Ag、乙型肝炎病毒e抗原(HBe Ag)、HBc Ab均陽性23例,其他30例;其配偶HBs Ag、乙型肝炎病毒表面抗體(HBs Ab)、HBe Ag、HBe Ab、HBc Ab均陰性34例。112例HBs Ag陽性女性中,HBs Ag、HBe Ab、HBc Ab均陽性75例,HBs Ag、HBe Ag、HBc Ab均陽性16例,其他21例;其配偶HBs Ag、HBs Ab、HBe Ag、HBe Ab、HBc Ab均陰性13例。28例RPR和TPPA均陽性中,夫妻雙方RPR和TPPA均陽性者9對。236例ALT異常男性中,HBs Ag、HBe Ag、HBc Ab均陽性14例;157例ALT異常女性中,HBs Ag、HBe Ag、HBc Ab均陽性7例。≥40歲HBSAg陽性檢出率大于20~29歲、30~39歲(P0.05);30~39歲ALT異常檢出率大于20~29歲(P0.05)。2014年HBSAg陽性檢出率小于2012年、2013年(P0.05);2014年ALT異常檢出率小于2012年(P0.05)。結(jié)論 HBV、TP在河北省備孕夫妻中均有一定的感染率,ALT異常也有相當(dāng)?shù)臋z出率,應(yīng)對檢出患者進(jìn)行優(yōu)生優(yōu)育指導(dǎo),以有效阻止HBV、TP、HIV在夫妻間的傳播和母嬰垂直傳播,減少出生缺陷、不良妊娠和傳染病的發(fā)生。
[Abstract]:Objective to investigate the infection status of hepatitis B virus (HBV), syphilis and human immunodeficiency virus (HIV) and the abnormal rate of alanine aminotransferase (alt) between husband and wife before pregnancy so as to take effective measures to prevent the transmission between husband and wife and vertical transmission from mother to child.
