脈搏波波速法無創(chuàng)血壓測量中多模量血壓計算模型研究

【摘要】:針對脈搏波波速法無創(chuàng)血壓測量中血壓計算模型建模困難和模型計算精度較低的問題,結(jié)合TPTT、ln(TPTT)及(1/TPTT)2等模量建立多模量血壓計算模型。首先,利用99名隨機(jī)測試者的實驗數(shù)據(jù)確定多模量模型參數(shù),并基于實驗數(shù)據(jù)計算各模型性能評價指標(biāo),其中多模量血壓計算模型擬合相關(guān)系數(shù)最大,為0.891,誤差方差最小,僅為6.1,實驗表明,多模量血壓計算模型具有更好的擬合效果和更低的計算誤差。然后,利用醫(yī)用水銀血壓計和自主設(shè)計的多模量血壓測量系統(tǒng)兩種方法采集另外36名隨機(jī)測試者的收縮壓和舒張壓數(shù)據(jù),并計算兩種方法采集數(shù)據(jù)間的相關(guān)參數(shù),其中收縮壓差值的絕對值d6 mm Hg,差值均值E_d=0.55 mm Hg,差值的標(biāo)準(zhǔn)差δ_d=2.98 mm Hg;舒張壓差值的絕對值d6 mm Hg,差值均值E_d=0.57 mm Hg,差值的標(biāo)準(zhǔn)差δ_d=3.42 mm Hg,完全符合美國醫(yī)療儀器促進(jìn)協(xié)會SP10-199中對電子血壓計測量差值8 mm Hg的要求。最后,采用Bland-Altman差值法,對兩種方法測量數(shù)據(jù)一致性進(jìn)行檢驗,發(fā)現(xiàn)舒張壓與收縮壓的95%一致性界限分別為(-5.3,6.4)和(7.2,-6.2),完全在臨床血壓測量可接受范圍之內(nèi),較好地證明多模量血壓計算模型用于無創(chuàng)血壓測量的有效性。研究結(jié)果表明,多模量血壓計算模型可以應(yīng)用于脈搏波波速法無創(chuàng)血壓測量。
[Abstract]:In view of the difficulty of modeling the blood pressure model and the low accuracy of the model in pulse wave velocity noninvasive blood pressure measurement, a multimode blood pressure calculation model was established by combining TPTT / TPTT and TPTT / TPTT / TPTT / TPTT / TPTT / TPTT / TPTT / TPTT / TPTT / TPTT / TPTT. First of all, the parameters of multiple modulus model are determined by using the experimental data of 99 random subjects, and the performance evaluation indexes of each model are calculated based on the experimental data. The fitting correlation coefficient of the multimodule blood pressure calculation model is the largest, which is 0.891and the error variance is the smallest. The experimental results show that the multimode blood pressure calculation model has better fitting effect and lower calculation error. Then, the systolic and diastolic blood pressure data of 36 random subjects were collected by using the medical mercury sphygmomanometer and the self-designed multimode blood pressure measurement system, and the relevant parameters between the two methods were calculated. The absolute value of systolic pressure difference is d6 mm Hg, the mean value of difference is E_d=0.55 mm Hg, the standard deviation of difference is 未 D / D = 2.98 mm Hg, the absolute value of diastolic pressure difference is d 6 mm Hg, the mean value of difference is E_d=0.57 mm Hg, and the standard deviation 未 D / D of difference is 3.42 mm Hg, which is fully in line with the promotion of American medical instruments. Association SP10-199 requirements for electronic sphygmomanometer measurement difference of 8 mm Hg. Finally, the Bland-Altman difference method was used to test the data consistency of the two methods. The 95% consistency limits of diastolic blood pressure and systolic blood pressure were found to be -5.3f6.4and 7.2mg-6.2respectively, which were completely within the acceptable range of clinical blood pressure measurement. The validity of multimode blood pressure calculation model in non-invasive blood pressure measurement is well proved. The results show that the multimode blood pressure calculation model can be applied to pulse wave velocity noninvasive blood pressure measurement.
