給氧負(fù)壓傷口治療在骨科慢性傷口中的可行性研究

【摘要】:目的評價負(fù)壓傷口治療聯(lián)合局部給氧對骨科慢性傷口的可行性及效果。方法對60例骨科創(chuàng)傷性慢性傷口患者隨機分為兩組,每組30例,對照組給予負(fù)壓傷口治療,連續(xù)10d。觀察組在負(fù)壓傷口治療基礎(chǔ)上聯(lián)合局部給氧2L/min,干預(yù)時間同對照組,觀察指標(biāo):2組治療0d和10d監(jiān)測傷口滲液pH、溫度、PUSH愈合計分、肉芽組織覆蓋率、傷口達到植皮條件所需時間、植皮成活率、愈合時間、治愈率。結(jié)果治療10d后,觀察組傷口滲液pH(7.22±0.16),低于對照組(7.50±0.26)(P0.01)。達到植皮條件所需時間(12.65±7.16)d,低于對照組(18.28±8.85)d,傷口床的溫度(34.02±0.41)℃,高于對照組(33.25±0.35)℃(P0.01)。觀察組肉芽組織覆蓋率(34.30±2.83)%高于對照組(27.27±3.54)%(P=0.00)。植皮成活率(93.22±1.91)%,高于對照組(83.21±1.86)%(P=0.00)。壓瘡愈合計分量表(pressure ulcer scale for healing,PUSH)計分結(jié)果差異無統(tǒng)計學(xué)意義(P0.05)。所有患者隨訪3個月,觀察組治愈率為86.67%(26/30),平均愈合時間為(35±15.5)d,對照組治愈率為53.33%(16/30),平均愈合時間為(49±23.4)d,差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論將負(fù)壓傷口治療與局部給氧聯(lián)合應(yīng)用于骨科慢性傷口能促進傷口愈合及肉芽組織生長,方法可行,效果優(yōu)于單純負(fù)壓治療。
[Abstract]:Objective to evaluate the feasibility and effect of negative pressure wound treatment combined with local oxygen on chronic orthopedic wound. Methods 60 cases of traumatic chronic wound in orthopedic department were randomly divided into two groups, 30 cases in each group. The control group was treated with negative pressure wound for 10 days. The observation group combined with local oxygen 2L / min on the basis of negative pressure wound treatment, the intervention time was the same as that of the control group.
