【摘要】：目的分析影響擇期手術(shù)患者貯存式自體血利用率的因素,探討其在擇期手術(shù)貯存式自體血采集中的指導(dǎo)意義。方法 2013年1-11月對(duì)本院106名貯存式自體輸血(PABD)患者和97名未備自體血患者貯存式自體血的利用進(jìn)行回顧性分析,分析術(shù)前Hb水平、術(shù)中預(yù)計(jì)出血量、自體血回輸率、異體血輸注及人均輸血量等,對(duì)PABD的利用效率進(jìn)行分析討論。結(jié)果 106名PABD患者中有101例(95.3%)自體血得以回輸,其中77例患者(72.6%)術(shù)中僅輸注自體血,26例(24.5%)因術(shù)中出血較多而又輸注異體血;5例(4.7%)自體血未回輸。當(dāng)患者術(shù)前Hb水平125 g/L時(shí),術(shù)中實(shí)際出血量800 m L時(shí),PABD組總的輸血率和輸血量均大于對(duì)照組(P0.05);出血量800 m L時(shí),2組患者總輸血量沒有差異,與術(shù)前Hb水平無關(guān)。結(jié)論患者術(shù)前Hb水平較高且術(shù)中出血量較少時(shí),實(shí)施PABD增加了總的輸血量和自體血的報(bào)廢率,對(duì)異體血的節(jié)約作用有限。
[Abstract]:Objective to analyze the factors affecting the storage type of autologous blood utilization in elective surgery patients and to explore the guiding significance of the storage type autologous blood collection in elective surgery. Methods a retrospective analysis was made on the use of storage type autologous blood in 106 PABD patients and 97 patients without autologous blood in 1-11 months of 2013. Hb level, intraoperative bleeding, autologous blood transfusion, allogeneic blood transfusion and per capita blood transfusion, the use efficiency of PABD was analyzed and discussed. Results of 106 PABD patients, 101 cases (95.3%) of autologous blood could be retransfused, of which 77 patients (72.6%) were only transfused with autologous blood, 26 cases (24.5%) had more bleeding and transfusion of allogeneic blood; 5 The total blood transfusion rate and blood transfusion amount in group PABD were greater than that of the control group (P0.05) when the actual amount of bleeding was 800 m L before operation, and the total blood transfusion rate and blood transfusion amount in the PABD group were higher than those of the control group (P0.05). When the amount of bleeding was 800 m L, the total blood volume of the 2 groups was not different from the preoperative Hb level. The implementation of the higher level of Hb and less bleeding during the operation was carried out in the patients before operation. (4.7%) the amount of blood transfusion and blood transfusion in PABD group were higher than that of the control group (P0.05). PABD increases the total blood transfusion and the rejection rate of autologous blood, which has limited effect on allogeneic blood.
