ICU與非ICU患者中心靜脈導(dǎo)管感染調(diào)查

【摘要】:目的比較ICU與非ICU住院患者中心靜脈導(dǎo)管感染的狀況,提出預(yù)防對策,以降低患者中心靜脈導(dǎo)管感染率。方法選擇2010年1月-2015年6月在醫(yī)院ICU和非ICU行中心靜脈導(dǎo)管的患者各200例,對疑有中心靜脈導(dǎo)管感染的患者進(jìn)行血液及中心靜脈導(dǎo)管尖端培養(yǎng),比較ICU和非ICU患者的中心靜脈導(dǎo)管感染率、置管部位、置管類型及病原菌分布。結(jié)果 ICU和非ICU患者血液、中心靜脈導(dǎo)管培養(yǎng)陽性率及導(dǎo)管細(xì)菌定植率比較差異均無統(tǒng)計(jì)學(xué)意義;普通導(dǎo)管和血液透析導(dǎo)管的感染率較高,分別為12.66%和6.67%,抗感染導(dǎo)管感染率為11.11%明顯較低;頸內(nèi)靜脈置管患者中心靜脈導(dǎo)管感染率較高為38.46%;ICU與非ICU患者病原菌檢出分布差異無統(tǒng)計(jì)學(xué)意義;ICU與非ICU患者留置中心靜脈導(dǎo)管1~7d的感染率9.00%,8~14d感染率為22.00%,≥15d感染率為35.00%,ICU與非ICU患者的置管時(shí)間比較差異無統(tǒng)計(jì)學(xué)意義。結(jié)論 ICU與非ICU患者的中心靜脈導(dǎo)管感染發(fā)病情況基本相同:抗感染導(dǎo)管的感染率較低,頸內(nèi)靜脈置管的感染率較高,病原菌以革蘭陽性菌為主。
[Abstract]:Objective to compare the status of central venous catheter infection in patients with ICU and non-ICU, and to put forward preventive measures to reduce the infection rate of central venous catheter. Methods from January 2010 to June 2015, 200 patients with central venous catheter were treated with central venous catheter in ICU and 200 patients with non-central venous catheter in hospital from January 2010 to June 2015. Blood and central venous catheter tip culture were performed in patients with suspected central venous catheter infection. The infection rate, catheterization site, type of catheterization and the distribution of pathogens in ICU and non-ICU patients were compared. Results there was no significant difference in the positive rate of blood, central venous catheter culture and bacterial colonization rate between ICU and non- patients, and the infection rate of common catheter and hemodialysis catheter was higher than that of normal catheter and hemodialysis catheter. The infection rate of anti-infective catheter was 11.11%, which was significantly lower than that of control group (12.66% and 6.67%, respectively). The infection rate of central venous catheter in patients with internal jugular vein catheterization was higher than that in ICU patients. There was no significant difference in the detection and distribution of pathogenic bacteria between ICU patients and non-ICU patients.
