【摘要】：目的 :探討血清可溶性血管內(nèi)皮生長(zhǎng)因子受體-1(soluble fms-like tyrosine kinase-1,s Flt-1)、胎盤生長(zhǎng)因子(placental growth factor,PLGF)水平及比值變化與輕度子癇前期發(fā)病關(guān)系。方法 :檢索萬方、CNKI、Pub Med、Embase和Cochrane收集1994—2016年國(guó)內(nèi)外公開發(fā)表的關(guān)于血清s Flt-1、PLGF水平及比值變化與輕度子癇前期關(guān)系的中英文病例對(duì)照研究,剔除不符合標(biāo)準(zhǔn)的文獻(xiàn),對(duì)納入的文章采用Review Manager 5.3軟件對(duì)結(jié)果進(jìn)行統(tǒng)計(jì)分析。結(jié)果:共納入文獻(xiàn)17篇,其中中文文獻(xiàn)14篇,英文文獻(xiàn)3篇。血清s Flt-1表達(dá)水平與輕度子癇前期的關(guān)系的異質(zhì)性檢驗(yàn)I~2=96%,P0.000 01,存在明顯的異質(zhì)性,合并效應(yīng)SMD=4.89(95%CI:3.60~6.17),P0.000 01,差異有統(tǒng)計(jì)學(xué)意義。血清PLGF表達(dá)水平與輕度子癇前期的關(guān)系的異質(zhì)性檢驗(yàn)I~2=93%,P0.000 01,存在明顯的異質(zhì)性,合并效應(yīng)SMD=-2.32(95%CI:-2.91~-1.72),P0.000 01,差異有統(tǒng)計(jì)學(xué)意義。輕度子癇前期組和正常對(duì)照組血清s Flt-1/PLGF比值異質(zhì)性檢驗(yàn)I~2=98%,P0.000 01,合并效應(yīng)SMD=1.95(95%CI:0.50~3.40),P0.000 01,差異有統(tǒng)計(jì)學(xué)意義。結(jié)論:PLGF與輕度子癇前期的發(fā)病具有密切關(guān)系,血清s Flt-1的表達(dá)水平與輕度子癇前期有關(guān),s Flt-1以及s Flt-1/PLGF比值可以作為輔助診斷輕度子癇前期的有效指標(biāo)。
[Abstract]:Objective: to investigate the relationship between the level and ratio of soluble fms-like tyrosine kinase-1 (Flt-1) and placental growth factor (PLGF) in patients with mild preeclampsia. Methods: a Chinese and English case-control study on the relationship between serum sFlt-1 PLGF level and its ratio to mild preeclampsia was collected from Cochrane and Pub MedEmbase from 1994 to 2016, and the substandard literature was excluded. The results were analyzed by Review Manager 5.3 software. Results: a total of 17 articles were included, including 14 in Chinese and 3 in English. Heterogeneity of the relationship between the expression of serum s Flt-1 and mild preeclampsia; there was significant heterogeneity in the expression of serum s Flt-1, and there was significant heterogeneity. The combined effect was 4. 8995% CI: 3. 60 ~ 6. 17% P 0. 000 01, and the difference was statistically significant. Heterogeneity test of the relationship between serum PLGF expression level and mild preeclampsia (P 0.00001), there was significant heterogeneity. The combined effect of SMD-2.32A95% CI: -2.91- 1.72P0.00001, the difference was statistically significant. The heterogeneity of serum s Flt-1/PLGF ratio between the mild preeclampsia group and the normal control group (P 0.00001) and the combined effect (SMD-1.95 / 95 CI: 0.50 / 3.40C / P 0.00001) were statistically significant. Conclusion there is a close relationship between the expression of serum s Flt-1 and mild preeclampsia, and the ratio of s Flt-1 and s Flt-1/PLGF can be used as an effective index for the diagnosis of mild preeclampsia.
