85株兒童感染金黃色葡萄球菌耐藥性及流行病學(xué)研究

【摘要】:目的了解安徽省各地區(qū)近年兒童感染金黃色葡萄球菌(SA)耐藥性及其流行病學(xué)特征,分析耐甲氧西林金黃色葡萄球菌(MRSA)的葡萄球菌盒式染色體(SCCmec)基因型分布特點(diǎn),為臨床感染和控制提供依據(jù)。方法臨床分離2010年~2013年每年9月非重復(fù)的兒童SA共85株,利用Microscan Walk-Away-40全自動(dòng)微生物分析儀進(jìn)行細(xì)菌鑒定和藥敏試驗(yàn),MRSA經(jīng)瓊脂倍比稀釋法進(jìn)行表型篩選、聚合酶鏈反應(yīng)(PCR)最終確定。多重PCR法檢測(cè)MRSA SCCmec基因分型。結(jié)果 85株SA中檢出19株MRSA和66株甲氧西林敏感金黃色葡萄球菌(MSSA),MRSA的檢出率為22.35%。MRSA檢出率最多的標(biāo)本是痰液及咽拭子,其次為分泌物。全部SA對(duì)青霉素、克林霉素完全耐藥,未發(fā)現(xiàn)萬(wàn)古霉素耐藥及中介的菌株。與MSSA藥敏結(jié)果比較,MRSA對(duì)16種常用抗菌藥物的耐藥性更高。MRSA SCCmec分型以SCCmecⅢ最多見(jiàn),共12株,其次為SCCmec I 5株,未分型2株,未發(fā)現(xiàn)SCCmecⅡ、Ⅳ、Ⅴ型。結(jié)論本地區(qū)兒童感染SA有不同的耐藥譜,兒童MRSA對(duì)同種抗菌藥物耐藥率均高于MSSA。兒童臨床分離的MRSA菌株其SCCmec基因型以Ⅲ型為主。臨床應(yīng)合理使用抗菌藥物,定期監(jiān)測(cè)耐藥性變遷,避免院內(nèi)傳播。
[Abstract]:Objective to investigate the drug resistance of children infected with Staphylococcus aureus (SAA) and its epidemiological characteristics in Anhui Province in recent years, and to analyze the distribution of SCCmec-genotypes of MRSAs resistant to methicillin-resistant Staphylococcus aureus. To provide the basis for clinical infection and control. Methods from 2010 to September 2013, 85 strains of children with non-repeated SA were isolated. Bacterial identification and drug sensitivity test were performed by Microscan Walk-Away-40 automatic microbiological analyzer. Phenotypic screening was performed by Agar doubling dilution method, and PCR was used to determine the phenotype. The genotyping of MRSA SCCmec was detected by multiple PCR method. Results among 85 strains of SA, 19 strains of MRSA and 66 strains of methicillin-sensitive Staphylococcus aureus were found to have the highest detectable rate of 22.35%.MRSA in sputum and pharynx swab, followed by secretion. All SA were completely resistant to penicillin and clindamycin, and no vancomycin resistant and intermediate strains were found.
