【摘要】：目的 探討替加環(huán)素耐藥肺炎克雷伯桿菌感染(Tigecycline-resistant Klebsiella pneumoniae, TRKp)的臨床特點(diǎn)、易感因素及防治措施。 方法 對(duì)2012年12月到2014年5月期間,我院各病區(qū)住院患者分離培養(yǎng)到TRKp并臨床診斷為相關(guān)系統(tǒng)感染的病例資料進(jìn)行系統(tǒng)性回顧性分析。 結(jié)果 共183例肺炎克雷伯桿菌感染患者,替加環(huán)素耐藥64例(35.0%)；耐藥者平均年齡60.5±17.6歲；中位住院時(shí)間37天。替加環(huán)素耐藥者中46例(71.9%)為肺部感染,37例(57.8%)耐藥前曾使用碳青霉烯類抗生素,57例(89.1%)留置導(dǎo)尿管,19例(29.7%)耐藥者合并有腦血管病。有25例(39%)耐藥者病情發(fā)生惡化及死亡。 結(jié)論 替加環(huán)素耐藥肺炎克雷伯桿菌的產(chǎn)生與住院時(shí)間、感染部位、抗生素的使用、侵入性操作及基礎(chǔ)疾病等均相關(guān),病死率高,臨床工作應(yīng)注意防范,以盡可能減少替加環(huán)素耐藥肺炎克雷伯桿菌感染的產(chǎn)生。
[Abstract]:Purpose

To investigate the clinical characteristics , susceptibility factors and control measures of tegastigmine - resistant Klebsiella pneumoniae ( TRK ) .

method

During the period from December 2012 to May 2014 , a systematic retrospective analysis was performed on the case data for the isolation and culture of the patient from each hospital in our hospital and the clinical diagnosis of the associated system infection .

Results

In total 183 cases of Klebsiella pneumoniae infection , 64 cases ( 35.0 % ) were resistant to tigecycline .
The mean age of drug - resistant was 60.5 鹵 17.6 years .
The median hospitalization time was 37 days . 46 of tigecycline - resistant patients ( 71.9 % ) were lung infections , 37 ( 57.8 % ) were resistant to antibiotics before drug resistance , 57 ( 89.1 % ) were indwelling urinary catheters , 19 ( 29 . 7 % ) resistant patients had cerebrovascular diseases .

Conclusion

The production of tigecycline - resistant Klebsiella pneumoniae is related to the hospitalization time , the site of infection , the use of antibiotics , the invasive operation and the underlying diseases , etc . , the mortality rate is high , the clinical work should pay attention to prevention , so as to minimize the production of tegastigecycline - resistant Klebsiella pneumoniae infection .
