【摘要】：目的觀察利拉魯肽對(duì)不同海拔地區(qū)糖調(diào)節(jié)受損肥胖患者內(nèi)皮功能障礙的影響。方法選擇經(jīng)彩色多普勒檢查血管內(nèi)皮功能障礙的糖調(diào)節(jié)受損肥胖患者98例,按居住地海拔高度分為低海拔(1 780~2 999米)組(n=52)和高海拔(3 000~3 719米)組(n=46)。全部患者使用利拉魯肽(起始劑量0.6 mg·d-1,皮下注射)治療3個(gè)月,治療前后檢測(cè)血糖,血脂,血壓,肝、腎功能等指標(biāo),使用全數(shù)字化高分辨率彩超測(cè)定肱動(dòng)脈血管內(nèi)皮依賴性舒張功能(FMD)。結(jié)果與治療前比較,治療后兩組肝、腎功能,空腹血糖,餐后血糖,血脂,血壓,腰臀比,糖化血紅蛋白和C反應(yīng)蛋白均有改善趨勢(shì),但無(wú)顯著差異(P0.05);兩組空腹胰島素和穩(wěn)態(tài)胰島素評(píng)價(jià)指數(shù)水平均顯著下降(P0.05),且低海拔組較高海拔組下降更為顯著(P0.05)。治療后兩組FMD均顯著高于治療前(P0.05),且低海拔組高于高海拔組(P0.05),低海拔組和高海拔組分別有28例(54%)和14例(30%)患者FMD轉(zhuǎn)為正常,兩組相比差異顯著(P0.05)。結(jié)論利拉魯肽對(duì)不同海拔地區(qū)糖調(diào)節(jié)受損肥胖患者的血管內(nèi)皮功能障礙及胰島素抵抗均有不同程度的改善作用,且對(duì)低海拔地區(qū)患者改善作用更為顯著。
[Abstract]:Objective to observe the effect of Lilaru peptide on endothelial dysfunction in obese patients with impaired glucose regulation at different altitudes. Methods 98 patients with impaired glucose regulation were examined by color Doppler. According to the altitude of the residence, the patients were divided into two groups: the low altitude group (1 780 m) and the high altitude group (3 000 ~ 3 719 m). All the patients were treated with liverrutin (initial dose 0.6 mg 路d ~ (-1) subcutaneously) for 3 months. Blood glucose, blood lipid, blood pressure, liver, liver, liver, liver and liver were measured before and after treatment. The endothelium-dependent diastolic function of brachial artery was measured by full-digital high-resolution color ultrasound. Results compared with those before treatment, liver, renal function, fasting blood glucose, postprandial blood glucose, blood lipid, blood pressure, waist-to-hip ratio were measured in two groups after treatment. Both glycosylated hemoglobin and C-reactive protein showed a trend of improvement. However, there was no significant difference between the two groups in the evaluation index of fasting insulin and steady-state insulin, and the levels of fasting insulin and steady-state insulin evaluation index in both groups were significantly lower than those in the high altitude group. After treatment, the FMD of the two groups was significantly higher than that of the pre-treatment group, and that of the low altitude group was higher than that of the low altitude group, and that of the low altitude group was higher than that of the high altitude group. In the high altitude group, there were 28 cases of P0.05A in the low altitude group and 14 cases in the high altitude group (30%). Conclusion Lilaru peptide can improve vascular endothelial dysfunction and insulin resistance in obese patients with impaired glucose regulation at different altitudes, especially in low altitude areas.
