【摘要】：目的分析《特殊情況緊急搶救輸血推薦方案》(簡稱《緊急輸血方案》)在醫(yī)院緊急醫(yī)療搶救工作中實施的效果及意義。方法根據(jù)《緊急輸血方案》的應(yīng)用范圍,從廣州市第一人民醫(yī)院2013-2015年46 061例輸血病例中,收集《緊急輸血方案》公布前后3年符合特殊情況緊急搶救輸血的524例病例做回顧性總結(jié)分析和比較,包括《緊急輸血方案》實施前后特殊情況緊急搶救輸血的一般及變化情況、ABO及Rh血型同型輸注與相容性輸注比例、輸血適應(yīng)證種類、病例的臨床科室分布、實驗室檢測處理時間以及輸血不良反應(yīng)發(fā)生率等。結(jié)果在《緊急輸血方案》的指導(dǎo)下,完成374例特殊情況需要緊急搶救輸血的臨床輸血病例,醫(yī)院輸血科及相關(guān)科室執(zhí)行緊急特殊輸血治療的能力和時間效率都有很大提高,尤其是《緊急輸血方案》公布前后處理ABO疑難血型患者緊急搶救輸血病例的實驗室處理時間(h)由1.37±0.21縮短至0.32±0.10(P0.05)、庫存不足導(dǎo)致需要不同型血液成分輸注的實驗室處理時間(h)由0.31±0.10縮短至0.22±0.10(P0.05),并體現(xiàn)在RhD陰性血患者緊急搶救的病例中。結(jié)論《緊急輸血方案》為輸血科和臨床科室提供了科學(xué)、規(guī)范、標(biāo)準(zhǔn)的輸血急救治療依據(jù)和指導(dǎo),使得搶救輸血能有效緩解患者病情和贏得治療時間。
[Abstract]:Objective to analyze the effect and significance of "Emergency Blood Transfusion Program for Special cases" (abbreviated as "Emergency Blood Transfusion Program") in hospital emergency medical treatment. Methods according to the scope of application of Emergency Blood Transfusion Program, From 46,061 cases of blood transfusion in the first people's Hospital of Guangzhou from 2013 to 2015, 524 cases of emergency blood transfusion according to special circumstances were collected before and after the publication of Emergency Blood Transfusion Program for retrospective analysis and comparison. Including the general and changes of emergency blood transfusion before and after the implementation of "Emergency Blood Transfusion Program", the proportion of ABO and Rh blood group transfusion and compatible transfusion, the indication of blood transfusion, the distribution of clinical departments of the cases. Results under the guidance of "Emergency Blood Transfusion Program", 374 cases of clinical blood transfusion requiring emergency rescue of blood transfusion were completed under the guidance of "Emergency Blood Transfusion Program". The ability and time efficiency of hospital blood transfusion department and related departments to carry out emergency and special transfusion treatment have been greatly improved. In particular, the laboratory treatment time before and after the publication of the Emergency Blood Transfusion Program for the treatment of ABO patients with difficult blood groups was shortened from 1.37 鹵0.21 to 0.32 鹵0.10 P0.05, resulting in a shortage of stocks leading to the need for laboratory treatment of different types of transfusion of blood components. The time was shortened from 0.31 鹵0.10 to 0.22 鹵0.10 (P0.05), which was reflected in the emergency rescue of patients with RhD negative blood. Conclusion the "Emergency Blood Transfusion Scheme" provides a scientific basis for blood transfusion department and clinical department. The standard and standard basis and guidance of first aid transfusion can effectively relieve the patient's condition and win treatment time.
