住院患者誤吸危險因素評估量表的設(shè)計和應(yīng)用

【摘要】:目的設(shè)計住院患者的誤吸危險因素評估量表,提高對誤吸反流的早期護(hù)理安全預(yù)警系統(tǒng),確保患者的住院安全。方法選取呼吸科收治的285例患者,其中誤吸患者34例作為觀察組,非誤吸患者251例作為對照組。對所有患者的基本資料進(jìn)行回顧性分析,建立危險因素評估表的具體條目,對各條目的分析采用醊2檢驗,對有意義變量采用logistics回歸分析,確定量表分值,采用受試者工作曲線(ROC)分析最佳截斷值,確定分值和風(fēng)險級別。結(jié)果既往誤吸史、體位、人工氣道、飲食方式、鼻飼方式等是發(fā)生誤吸的主要危險因素。ROC曲線分析顯示,分值3分時對發(fā)生誤吸診斷的敏感性為90.9%、特異性為85.0%。結(jié)論制定誤吸高風(fēng)險的評估量表,可以讓臨床醫(yī)務(wù)人員迅速而準(zhǔn)確地評估住院患者的誤吸風(fēng)險,對有風(fēng)險的患者及時采取針對性的預(yù)防措施,以減少誤吸的發(fā)生,改善患者的臨床結(jié)局,提高醫(yī)療和護(hù)理質(zhì)量。
[Abstract]:Objective to design the risk factor assessment scale for inpatients with misaspiration, to improve the early nursing safety warning system for mis-aspiration reflux, and to ensure the safety of hospitalization. Methods 285 patients treated in respiratory department were selected. Among them, 34 cases were used as observation group and 251 cases as control group. The basic data of all patients were analyzed retrospectively, and the specific items of risk factor assessment table were established. The significant variables were analyzed by logistics regression analysis, the scale score was determined, the optimal truncation value was analyzed by using the operating curve of subjects, and the score and risk level were determined. The ROC curve analysis showed that the sensitivity and specificity of nasal feeding were 90.9 and 85.0 respectively. Conclusion the evaluation scale for high risk of false aspiration was developed, and the ROC curve showed that the sensitivity was 90.9 and the specificity was 85.0 when the score was 3. It is possible for clinical medical staff to evaluate the risk of aspiration miscarriage quickly and accurately, and to take timely preventive measures for the patients at risk, so as to reduce the occurrence of false aspiration, improve the clinical outcome of patients, and improve the quality of medical treatment and nursing care.
