【摘要】：目的探討不同劑量烏司他丁對(duì)老年膿毒癥患者腸黏膜屏障功能保護(hù)作用的影響,為臨床應(yīng)用提供依據(jù)。方法將入選的老年膿毒癥患者隨機(jī)分為小劑量治療組(n=39)和大劑量治療組(n=39)。小劑量治療組給予10萬(wàn)U靜脈注射每8 h 1次,大劑量治療組給予20萬(wàn)U靜脈注射每8 h 1次。分別于入住ICU當(dāng)時(shí)(D0)與入住ICU后第2、3、7天(D2、D3、D7)測(cè)定血清腸脂肪酸結(jié)合蛋白(IFABP)、二胺氧化酶(DAO)、D-乳酸(DLac)水平變化,并記錄患者APACHEⅡ評(píng)分、ICU住院時(shí)間及28 d生存情況。結(jié)果 (1)入科時(shí)兩組患者APACHEⅡ評(píng)分、血漿IFABP、DAO、D-Lac水平差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。(2)經(jīng)治療后,兩組患者血漿IFABP、DAO、D-Lac水平均有所下降,但大劑量治療組下降趨勢(shì)表現(xiàn)得更加明顯。從D3觀察點(diǎn)開始,大劑量治療組血漿IFABP、DAO、D-Lac水平均明顯低于小劑量治療組(P0.05)。(3)經(jīng)治療后,大劑量治療組APACHEⅡ評(píng)分及ICU住院時(shí)間均低于小劑量治療組(P0.05)。(4)兩組患者28 d死亡率差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論大劑量烏司他丁對(duì)老年膿毒癥患者腸屏障功能的保護(hù)作用更加明顯,能明顯改善老年膿毒癥患者的預(yù)后,減少ICU住院時(shí)間,但對(duì)降低28 d死亡率無(wú)明顯作用。
[Abstract]:Objective to investigate the effects of different doses of ulinastatin on intestinal mucosal barrier function in elderly patients with sepsis. Methods the geriatric sepsis patients were randomly divided into small dose treatment group and high dose treatment group. The low dose treatment group was given 100,000 U intravenously every 8 h, and the low dose treatment group was given 100,000 U intravenously once every 8 hours. 200,000 U was injected intravenously every 8 hours in the high-dose treatment group. The levels of serum fatty acid binding protein (ICU) and diamine oxidase (DAOX) were measured at the time of admission to ICU (D0) and the second day after admission to ICU (D7).
