攜氧代血漿擴(kuò)容及攜氧功能初步動(dòng)物實(shí)驗(yàn)研究

【摘要】:目的建立大鼠重度失血性休克模型和大鼠失血性休克-腸系膜微循環(huán)模型,用以探討攜氧代血漿的擴(kuò)容和攜氧功能。方法將20只SD大鼠失血至全身血容量的60%,造成重度失血性休克大鼠模型后,隨機(jī)均分為6%羥乙基淀粉(HES)組和攜氧代血漿組做相應(yīng)樣品的等容量輸注復(fù)蘇。分別在造模前、休克、輸液末以及輸液后1 h觀測(cè)大鼠平均動(dòng)脈壓(MAP)、股動(dòng)脈取血測(cè)定血?dú)?檢測(cè)組織氧分壓(PtO_2)。另取20只大鼠建立失血性休克-腸系膜微循環(huán)模型,觀察、記錄大鼠造模前基礎(chǔ)值、休克、輸液末以及輸液后1 h微血管直徑的變化和血流狀態(tài)。結(jié)果 HES組與攜氧代血漿組比較,輸液末微靜脈直徑(μm)與微動(dòng)脈直徑(μm)分別為:31.23±4.96 vs 31.86±5.43、19.46±1.47 vs19.40±2.10(P0.05),輸液后血流狀態(tài)均得到恢復(fù);輸液后1 h的MAP(mm Hg)為:90.70±3.83 vs 98.90±9.18(P0.05);組織氧分壓(mm Hg)為:輸液末7.03±1.47 vs 15.32±2.34(P0.05),輸液后1 h 5.53±2.39 vs 8.90±2.01(P0.05);2組大鼠血?dú)庵笜?biāo)在各時(shí)間點(diǎn)相近(P0.05)。結(jié)論攜氧代血漿能夠改善大鼠失血性休克所致的微循環(huán)障礙,具有與HES相似的血管擴(kuò)容作用,并且在大鼠休克復(fù)蘇初期恢復(fù)組織氧分壓的效果優(yōu)于HES。
[Abstract]:Objective to establish a rat model of severe hemorrhagic shock and a rat model of hemorrhagic shock and mesenteric microcirculation. Methods the blood loss of 20 SD rats was increased to 60% of the systemic blood volume, and the model of severe hemorrhagic shock was established in rats. The rats were randomly divided into 6% hydroxyethyl starch (HES) group and oxygen-carrying plasma group for isometric infusion and resuscitation. The mean arterial pressure (map) was measured before, after shock, at the end of infusion and 1 hour after infusion. Blood was collected from femoral artery to determine blood gas. In addition, 20 rats were taken to establish hemorrhagic shock mesenteric microcirculation model. Results the diameter of microvein (渭 m) and the diameter of arteriole (渭 m) were 31.23 鹵4.96 vs 31.86 鹵5.43 鹵19.46 鹵1.47 vs19.40 鹵2.10P0.05in HES group and those in oxygen-carrying plasma group, respectively. At 1 hour after infusion, the MAP(mm Hg of the rats was: 1: 90.70 鹵3.83 vs 98.90 鹵9.18m P0.05A; the tissue oxygen partial pressure was 7.03 鹵1.47 vs 15.32 鹵2.34p0.05n; the blood gas index of rats in the group of 1 h after infusion was 5.53 鹵2.39 vs 8.90 鹵2.01P0.05U. Conclusion the oxygen-carrying substitute plasma can improve the microcirculation disturbance caused by hemorrhagic shock in rats. The effect of vasomascularization was similar to that of HES, and the effect of recovering tissue oxygen partial pressure in the early stage of shock resuscitation was better than that of HES.
