REP-PCR對醫(yī)護(hù)人員鼻腔中鮑曼不動桿菌分離株的溯源分析

【摘要】:目的對醫(yī)護(hù)人員鼻腔中的鮑曼不動桿菌進(jìn)行分離鑒定,對其來源和傳播途徑進(jìn)行分析。方法用棉拭子采集10名醫(yī)護(hù)人員和10名非醫(yī)護(hù)人員(大學(xué)生)鼻腔樣品,分離鑒定非重復(fù)鮑曼不動桿菌,采用K-B法檢測其對13種常用抗生素的敏感性;利用多重PCR擴(kuò)增OXA型碳青霉烯酶耐藥基因;應(yīng)用基因外重復(fù)回文序列REP-PCR對分離株進(jìn)行分子多樣性分析。結(jié)果共分離19株鮑曼不動桿菌,其中分離自醫(yī)護(hù)人員鼻腔12株,分離自非醫(yī)護(hù)人員鼻腔7株。K-B法檢測Ab的耐藥性,醫(yī)護(hù)人員鼻腔分離株對環(huán)丙沙星(CIP)、亞胺硫霉素(IPM)、頭孢曲松(CRO)、環(huán)磷酰胺(CTX)和新諾明(SXT)耐藥率均≥41.7%,非醫(yī)護(hù)人員分離株耐藥率均≤3%;耐藥基因檢測19株菌中都攜帶OXA-51基因,且醫(yī)護(hù)人員鼻腔分離株攜帶OXA-23菌株數(shù)多于非醫(yī)護(hù)人員鼻腔分離株。另外兩種未檢出耐藥基因(OXA-24和OXA-58)。REP-PCR顯示,醫(yī)護(hù)人員鼻腔分離株、非醫(yī)護(hù)人員鼻腔分離株Ab的8種基因型(A-H)中同源性≥90%的有33株菌,占94.29%(33/35)。結(jié)論鮑曼不動桿菌不僅能通過直接接觸傳播,也可能存在氣源性傳播途徑。
[Abstract]:Objective to isolate and identify Acinetobacter baumannii from the nasal cavity of medical staff and analyze its origin and transmission. Methods 10 medical staff and 10 non-medical personnel (college students) nasal cavity samples were collected with cotton swab. Non-repeated Acinetobacter baumannii was isolated and identified. The susceptibility of Acinetobacter baumannii to 13 common antibiotics was detected by K-B method, and the OXA carbapenem resistance gene was amplified by multiple PCR. Results A total of 19 strains of Acinetobacter baumannii were isolated, 12 of them were isolated from the nasal cavity of medical staff, and 7 strains were isolated from the nasal cavity of non-medical staff.
