【摘要】：目的探討引起ABO血型正反定型不相符的不規(guī)則抗體特異性及其免疫球蛋白類型。方法采用微柱凝膠法對(duì)患者進(jìn)行ABO和Rh D血型鑒定,對(duì)ABO血型正反定型不相符的血標(biāo)本再采用傳統(tǒng)試管法進(jìn)行復(fù)檢,并采用微柱凝膠法進(jìn)行不規(guī)則抗體篩選、特異性鑒定、抗體效價(jià)及免疫球蛋白類型檢測(cè)。結(jié)果在引起ABO血型正反定型不相符的56例不規(guī)則抗體中,MNS血型系統(tǒng)19例(33.9%)、P血型系統(tǒng)1例(1.8%)、Lewis血型系統(tǒng)15例(26.8%)、Kidd血型系統(tǒng)4例(7.1%)、Rh血型系統(tǒng)10例(17.9%)、冷自身抗體7例(12.5%)。免疫球蛋白類型為Ig M型42例(75.0%),抗體效價(jià)為1∶16~1∶8192;Ig G型10例(17.9%),抗體效價(jià)為1∶128~1∶1024;Ig M、Ig G混合型4例(7.1%),抗體效價(jià)為1∶32~1∶256。37℃反應(yīng)性:±~4~+。結(jié)論 Ig M型不規(guī)則抗體是引起ABO血型正反定型不相符的主要原因,高效價(jià)的Ig G型不規(guī)則抗體也可引起ABO血型正反定型不相符,在ABO血型鑒定中必須正反定型,同時(shí)進(jìn)行不規(guī)則抗體篩選才能保證檢測(cè)結(jié)果的準(zhǔn)確性和輸血安全。
[Abstract]:Objective to investigate the specificity of irregular antibodies and their immunoglobulin types which cause the incompatibility of positive and negative typing of ABO blood groups. Methods the ABO and Rh D blood groups of patients were identified by microcolumn gel method. The blood samples with different positive and negative typing of ABO blood group were re-examined by the traditional test tube method, and the irregular antibody was screened by microcolumn gel method, and the specificity was identified. Results among 56 cases of irregular antibodies that caused the inconsistency of positive and negative typing of ABO blood group, 19 cases were found to have MNS blood group system, 19 cases were found to have a 33. 9% P blood group system and 1 case was found to have a 1. 8% ABO blood group system. 15 cases were found to have a blood group system of 26. 8% and 4 cases of Rh blood group. The antibody titer of 10 cases was 17.9%, and that of cold autoantibody 7 cases was 12.55.The immunoglobulin type was Ig M type in 42 cases. The antibody titer was 1 16: 1: 81922 Ig G type in 10 cases, and the antibody titer was 1: 1281% 1024Ig mg G mixed type 4 cases. The titer of antibody was 1: 3232: 1 256.37 鈩，
