2012-2015年某傳染病醫(yī)院血流感染細(xì)菌構(gòu)成及耐藥性分析

【摘要】:目的了解全院血流感染分離菌的臨床分布及耐藥性,為臨床診斷及治療提供依據(jù)。方法采用Bac T/Alert 3D240全自動(dòng)血培養(yǎng)儀對(duì)全院2012年至2015年收集的9 923份血液標(biāo)本進(jìn)行培養(yǎng),采用VITEK2 COMPACT 60對(duì)分離的細(xì)菌進(jìn)行鑒定和藥敏試驗(yàn)。結(jié)果 9 923份血標(biāo)本中,656份檢出微生物,陽(yáng)性率6.6%。共分離出658株病原菌,其中革蘭陽(yáng)性菌占41.2%;革蘭陰性菌占37.1%;真菌占21.7%。病原菌種類主要為凝固酶陰性葡萄球菌(24.2%)、新型隱球菌(14.9%)、大腸埃希菌(13.5%)、沙門菌屬(6.7%)、馬爾尼菲青霉菌(5.8%)。分離的大腸埃希菌和肺炎克雷伯菌對(duì)氨芐西林的耐藥率分別為88.8%和100.0%,兩者ESBLS的檢出率分別為52.8%和11.1%;而凝固酶陰性葡萄球菌和金黃色葡萄球菌對(duì)青霉素的耐藥率最高,分別為96.6%和100.0%,二者頭孢西丁篩查陽(yáng)性率分別為85.8%和13.6%。結(jié)論全院血流感染病原菌以凝固酶陰性葡萄球菌為主,新型隱球菌、大腸埃希菌和沙門菌屬的檢出率也較高,應(yīng)予以高度重視。
[Abstract]:Objective to investigate the clinical distribution and drug resistance of blood stream infection isolates in our hospital, and to provide evidence for clinical diagnosis and treatment. Methods Bac T / Alert 3D240 automatic blood culture instrument was used to culture 9 923 blood samples collected from 2012 to 2015 in our hospital. VITEK2 COMPACT 60 was used to identify the isolated bacteria and drug sensitivity test. Results 656 microbes were detected in 9 923 blood samples, and the positive rate was 6.6. 658 strains of pathogenic bacteria were isolated. Gram-positive bacteria accounted for 41.2%; Gram-negative bacteria 37.1%; fungi 21.7.The main pathogens were coagulase negative staphylococci (24.22cm), Cryptococcus neoformans 14.9m, Escherichia coli 13.5m, Salmonella 6.7m, Penicillium marneffei 5.80.The isolated Escherichia coli was isolated. The resistance rates of Klebsiella pneumoniae and Klebsiella pneumoniae to ampicillin were 88.8% and 100.00.The detection rate of ESBLS was 52.8% and 11.1, respectively, while coagulase-negative staphylococci and Staphylococcus aureus had the highest resistance to penicillin. The positive rates of cefoxitin screening were 85.8% and 13.60.Conclusion coagulase negative staphylococcus was the main pathogen of blood stream infection in our hospital, and Cryptococcus neoformans, Escherichia coli and Salmonella were also detected. We should attach great importance to it.
