【摘要】：目的探討被動(dòng)抬腿試驗(yàn)(PLR)聯(lián)合超聲檢測(cè)心輸出量(CO)方法對(duì)老年休克患者容量反應(yīng)性預(yù)測(cè)的可行性。方法采取前瞻性觀察研究方法,選擇萬(wàn)州區(qū)第五人民醫(yī)院ICU 2013年1月至2015年9月老年休克患者38例,先后行PLR及補(bǔ)液療法。按補(bǔ)液療法后超聲檢測(cè)CO增加百分比大于或等于10%為對(duì)容量治療有反應(yīng)組,否則為無(wú)反應(yīng)組。結(jié)果 38例患者行45例次補(bǔ)液療法,有反應(yīng)組13例,無(wú)反應(yīng)組32例。有反應(yīng)組PLR及補(bǔ)液后CO明顯增加(P0.05),兩組數(shù)值△COPLR為(8.36±4.02)%,ΔCO補(bǔ)液為(8.59±3.53)%;觀察有反應(yīng)組ΔCOPLR與ΔCO補(bǔ)液兩組變量呈正相關(guān)(r=0.815,P=0.01)。ΔCOPLR預(yù)測(cè)容量反應(yīng)性的AUC值為0.854。當(dāng)閾值ΔCOPLR≥15%時(shí),預(yù)測(cè)容量反應(yīng)性敏感性87%,特異性91%。結(jié)論 PLR聯(lián)合超聲對(duì)CO檢測(cè)可有效預(yù)測(cè)容量反應(yīng)性,可作為一種無(wú)創(chuàng)方法指導(dǎo)老年患者休克治療。
[Abstract]:Objective to investigate the feasibility of passive leg lifting test (PLR) combined with echocardiography in the prediction of volumetric reactivity in elderly patients with shock. 38 elderly patients with shock from January 2013 to September 2015 in 5th people's Hospital of Wanzhou District were treated with PLR and rehydration therapy. The percentage of CO increase detected by ultrasound after fluid resuscitation was greater than or equal to 10% as the group with response to volume therapy. Results Thirty eight patients were treated with 45 times of rehydration therapy, and 13 cases in the response group. In the non-response group, PLR and CO increased significantly (P 0.05), the COPLR values of the two groups were 8.36 鹵4.02 and 8.59 鹵3.53.The positive correlation was observed between 螖 COPLR and 螖 CO rehydration, and the AUC value of 螖 COPLR was 0.854 when the threshold value of 螖 COPLR 鈮，
