【摘要】：目的探討3.0T MR髖關(guān)節(jié)造影(MRA)在髖臼唇撕裂診斷中的價值。方法回顧性分析74例髖關(guān)節(jié)疾病患者的髖關(guān)節(jié)鏡及術(shù)前髖關(guān)節(jié)MRA資料。以關(guān)節(jié)鏡檢查結(jié)果為金標(biāo)準(zhǔn),評估3.0T MRA診斷髖臼前唇和上唇撕裂的敏感度、特異度、陽性預(yù)測值、陰性預(yù)測值和準(zhǔn)確率,并比較MRA診斷髖臼前唇和上唇撕裂的差異。結(jié)果 74例患者的75個髖關(guān)節(jié)中,關(guān)節(jié)鏡證實(shí)61處前唇撕裂,29處上唇撕裂。2名醫(yī)師(醫(yī)師1和醫(yī)師2)采用MRA診斷髖臼前唇撕裂的敏感度、特異度、陽性預(yù)測值、陰性預(yù)測值和準(zhǔn)確率分別為90.16%(55/61)和91.80%(56/61)、78.57%(11/14)和85.71%(12/14)、94.83%(55/58)和96.55%(56/58)、64.71%(11/17)和70.59%(12/17)、88.00%(66/75)和90.67%(68/75),診斷上唇撕裂的敏感度、特異度、陽性預(yù)測值、陰性預(yù)測值和準(zhǔn)確率分別為89.66%(26/29)和93.10%(27/29)、91.30%(42/46)和95.65%(44/46)、86.67%(26/30)和93.10%(27/29)、93.33%(42/45)和95.65%(44/46)、90.67%(68/75)和94.67%(71/75)。2名醫(yī)師診斷髖臼前唇和上唇撕裂的一致性好(Kappa值分別為0.924和0.916)。MRA診斷髖臼上唇撕裂的陰性預(yù)測值高于前唇,差異有統(tǒng)計(jì)學(xué)意義(P=0.01),而敏感度、特異度、陽性預(yù)測值和準(zhǔn)確率的差異均無統(tǒng)計(jì)學(xué)意義(P均0.05)。結(jié)論 3.0T髖關(guān)節(jié)MRA是診斷髖臼唇撕裂的可靠影像方法。
[Abstract]:Objective to investigate the effect of 3. 0T Mr hip arthrography (MRAA). Methods the MRA data of 74 patients with hip diseases were analyzed retrospectively. The results of arthroscopy were taken as the golden standard. To evaluate the sensitivity, specificity, positive predictive value, negative predictive value and accuracy of 3.0T MRA in the diagnosis of acetabular anterior lip and upper lip tears. The difference of MRA in the diagnosis of acetabular anterior lip and upper lip tear was compared. Results 61 anterior lip tears were confirmed by arthroscopy in 75 of 74 patients. The sensitivity, specificity and positive predictive value of 29 upper lip tears (physician 1 and physician 2) were detected by MRA. The negative predictive value and the accuracy rate were 90.16 / 55 / 61) and 91.80 / 56 / 68.57 / 11 / 14 and 85.71 / 12 / 14, respectively. 94.83and 96.555.56 / 58 / 64.71) and 70.5912 / 17). The sensitivity, specificity and positive predictive value of the diagnosis of upper lip tear were 88.00% 66 / 75) and 90.67%. The Kappa values were 0.924 and 0.916, respectively. The negative predictive value of MRA in the diagnosis of acetabular upper lip tear was higher than that of the anterior lip. The difference was statistically significant (P < 0.01), but sensitivity and specificity. There was no significant difference in positive predictive value and accurate rate (P < 0.05). Conclusion 3.0T MRA is a reliable imaging method for the diagnosis of acetabular lip tear.
