【摘要】：目的調(diào)查耐β-內(nèi)酰胺類肺炎克雷伯菌(DRKP)常見耐藥元件的攜帶情況和菌株間的親緣性。方法收集2014年1-12月醫(yī)院住院患者痰液中分離到的30株DRKP,采用PCR分析17種β-內(nèi)酰胺類、6種氨基糖苷類獲得性耐藥基因以及4種可移動遺傳元件遺傳標記,并對檢測結(jié)果作樣本聚類分析。結(jié)果 30株DRKP對β-內(nèi)酰胺類(頭孢唑林、頭孢他啶、頭孢呋辛、頭孢噻肟等)、氨基糖苷類(慶大霉素、阿米卡星等)藥物及環(huán)丙沙星均耐藥,耐藥率均為100.0%,對亞胺培南與美羅培南均敏感,耐藥率均為0;每株DRKP均檢出β-內(nèi)酰胺類和氨基糖苷類藥物獲得性耐藥基因及MGEs標記;樣本聚類分析提示,30株DRKP有5組呈克隆傳播,存在醫(yī)院感染。結(jié)論β-內(nèi)酰胺酶blaTEM、blaDHA基因,氨基糖苷類修飾酶aac(3)-Ⅱ、aph3′-Ⅰ基因,MGEs標記intⅠ1、IS903在DRKP中流行,是導(dǎo)致該組菌對多種抗菌藥物耐藥的重要原因,基于獲得性耐藥基因與可移動遺傳元件標記檢測結(jié)果的樣本聚類分析是監(jiān)控醫(yī)院感染實用手段。
[Abstract]:Objective to investigate the 尾 -lactam resistant Klebsiella pneumoniae DRKP. Methods 30 strains of DRKP isolated from sputum of hospitalized patients from 2014 were collected. PCR was used to analyze 17 尾-lactams, 6 aminoglycoside acquired resistance genes and 4 transportable genetic elements. Results 30 strains of DRKP were treated with 尾 -lactam (cefazolin, ceftazidime, cefuroxime, cefotaxime, etc.) and aminoglycoside (gentamicin).
