【摘要】：目的評(píng)價(jià)擴(kuò)散加權(quán)成像(DWI)對(duì)預(yù)測肝細(xì)胞肝癌微血管浸潤的價(jià)值。方法搜集46例肝癌患者的DWI資料,測量腫瘤表觀擴(kuò)散系數(shù)(ADC)。比較微血管浸潤陽性組和微血管浸潤陰性組肝癌的ADC值,并進(jìn)一步采用受試者工作特征曲線(ROC)分析ADC評(píng)價(jià)微血管浸潤檢測的診斷性能。結(jié)果 46例肝癌患者中24例微血管浸潤陽性,22例微血管浸潤陰性。微血管浸潤陽性組的肝癌的ADC值低于微血管浸潤陰性組,并具有統(tǒng)計(jì)學(xué)差異(1.22×10-3mm2/s,1.54×10-3mm2/s,P=0.0004)。應(yīng)用ADC判斷微血管浸潤的曲線下面積(AUC)為0.78。以≤1.33×10-3mm2/s為界值判斷肝癌存在微血管浸潤的敏感性為75.0%,特異性為68.2%,陽性預(yù)測值為72.0%,陰性預(yù)測值為71.4%。結(jié)論 ADC值對(duì)于預(yù)測肝癌存在微血管浸潤有潛在的價(jià)值。
[Abstract]:Objective to evaluate the value of diffusion-weighted imaging (DWI) in predicting microvascular infiltration in hepatocellular carcinoma (HCC). Methods DWI data of 46 patients with HCC were collected. To measure the apparent diffusion coefficient of tumor and compare the ADC value of HCC in the positive microvessel infiltrating group and the negative microvascular infiltrating group. ADC was used to evaluate the diagnostic performance of microvessel infiltration. Results 24 of 46 patients with liver cancer were positive for microvascular infiltration.
