【摘要】：目的本研究旨在了解老年2型糖尿病患者的醫(yī)學(xué)應(yīng)對方式現(xiàn)狀,找出相關(guān)影響因素,以引起臨床護(hù)理工作者的關(guān)注,為老年2型糖尿病患者的護(hù)理工作提供一定理論依據(jù)。方法本研究采用便利抽樣法,于2014年3月至2014年7月,在錦州市、承德市各1所三級甲等醫(yī)院內(nèi)分泌科室共選取228例老年2型糖尿病患者。應(yīng)用中文版醫(yī)學(xué)應(yīng)對問卷(Medical Coping Modes Questionnaire,MCMQ)進(jìn)行調(diào)查。利用SSPS17.0對所得數(shù)據(jù)進(jìn)行統(tǒng)計(jì)學(xué)分析,分別利用獨(dú)立樣本t檢驗(yàn),F檢驗(yàn)和多重線性逐步回歸分析的方法,來探討老年2型糖尿病患者的醫(yī)學(xué)應(yīng)對現(xiàn)狀及其影響因素。結(jié)果1、老年2型糖尿病患者中傾向“面對”者136例(59.65%),傾向“回避”者70例(30.70%),傾向“屈服”者例22例(9.55%)。2、不同文化程度、地域、醫(yī)療負(fù)擔(dān)方式、病程的老年2型糖尿病患者在醫(yī)學(xué)應(yīng)對問卷各維度上的得分,均存在統(tǒng)計(jì)學(xué)意義的差異(P0.05)。3、老年2型糖尿病患者醫(yī)學(xué)應(yīng)對問卷各維度影響因素的多重線性逐步回歸分析顯示:(1)進(jìn)入“面對”維度回歸方程的有文化程度、病程、醫(yī)療負(fù)擔(dān)方式、性別(F=294.82,P0.05);(2)進(jìn)入“回避”維度回歸方程的有文化程度、病程、地域、醫(yī)療負(fù)擔(dān)方式(F=191.35,P0.05);(3)進(jìn)入“屈服”維度回歸方程的僅有文化程度(F=580.12,P0.05)。結(jié)論1、老年2型糖尿病患者醫(yī)學(xué)應(yīng)對問卷中的得分積極應(yīng)對得分低于全國常模,消極應(yīng)對得分高于全國常模。2、不同文化程度、地域、醫(yī)療負(fù)擔(dān)方式、病程的老年2型糖尿病患者所采取的醫(yī)學(xué)應(yīng)對方式各不相同。其中,文化程度高、有醫(yī)療保障、病程短者更趨向于積極面對疾病,文化程度低、無醫(yī)療保障、病程長者則趨向于消極應(yīng)對;從整體上看,相比于錦州地區(qū)而言,承德地區(qū)患者的積極應(yīng)對水平較高。3、老年2型糖尿病患者的醫(yī)學(xué)應(yīng)對問卷各維度得分中,對“面對”維度得分影響有意義的是文化程度、病程、醫(yī)療負(fù)擔(dān)方式、性別;對“回避”維度得分影響有意義的是文化程度、病程、地域、醫(yī)療負(fù)擔(dān)方式;對“屈服”維度得分影響有意義的是文化程度。
[Abstract]:Objective to investigate the current situation of medical coping style in elderly patients with type 2 diabetes mellitus and to find out the relevant influencing factors so as to attract the attention of clinical nursing workers. Methods convenience sampling method was used in this study, from March 2014 to July 2014, in Jinzhou City. 228 elderly patients with type 2 diabetes were selected from the Department of Endocrinology, 1 Grade 3A Hospital in Chengde City. The Chinese version of Medical coping questionnaire was used (. Medical Coping Modes Questionnaire. SSPS17.0 was used to analyze the data, and the independent sample t-test F test and multiple linear stepwise regression analysis were used. Results 1 136 elderly patients with type 2 diabetes were prone to "face" and 59.65). There were 70 cases with "avoidance" and 22 cases with "yield" with different education level, region and medical burden. There were statistically significant differences in the scores of medical coping questionnaire in the aged type 2 diabetes patients with the course of disease (P0.050.3.). Multiple linear stepwise regression analysis of the influencing factors of medical coping questionnaire for elderly patients with type 2 diabetes showed that: 1) entering the "face" dimension regression equation had the education level, course of disease, and the way of medical burden. FV 294.82 (P0.05); (2) the students who entered the regression equation of "avoidance" dimension included education level, course of disease, region, and medical burden mode (F191.35 / P0.05); (3) the education level of the regression equation of "yield" dimension is only FY580.12 / P0.050.Conclusion 1. The score of positive coping in the medical coping questionnaire of elderly type 2 diabetes patients was lower than that of the national norm, and the score of negative coping was higher than that of the national norm. The course of the elderly type 2 diabetes patients adopted different medical coping styles. Among them, the high level of education, there is medical security, short course of disease more tend to face the disease, low level of education, no medical security. The duration of the elderly tend to negative coping; On the whole, compared with Jinzhou area, Chengde patients' positive coping level is higher. The scores of medical coping questionnaire of elderly type 2 diabetes patients are in each dimension. The significant influence on the dimension score of "face" is the education level, the course of disease, the way of medical burden, the sex; The significant influence on the dimension score of "avoidance" is the degree of education, the course of disease, the region and the way of medical burden. It is the level of education that makes sense to the score of the subjugation dimension.
